Summary, etc. Purpose: Labor pain is a significant challenge in obstetric care, influencing the physical and psychological well-being of women during childbirth. Effective management of labor pain is crucial for ensuring a positive childbirth experience and promoting maternal and neonatal health. This thesis investigates the comparative efficacy and safety of two analgesics, paracetamol and meperidine, in relieving labor pain among primiparous women.<br/>Methods: The study was conducted as a randomized controlled trial at Al Kasr Al Aini University Hospital, including 112 primiparous women with term singleton pregnancies. Participants were randomly assigned to receive either intravenous paracetamol or intramuscular meperidine during the active phase of labor.<br/>The primary outcomes measured were pain intensity, duration of labor stages, and neonatal respiratory outcomes. Secondary outcomes included the need for further analgesia, maternal side effects, and neonatal Apgar scores.<br/>Results: Results demonstrated that both paracetamol and meperidine effectively reduced labor pain. The mean VAS score for the paracetamol group was 5.59 ± 1.49, while the meperidine group had a mean VAS score of 5.34 ± 1.48, with no statistically significant difference between the groups (p = 0.37). The duration of the active stage of labor was similar between the two groups, with Group I averaging 5.76 ± 1.52 hours and Group II averaging 5.71 ± 1.54 hours (p = 0.88). The second stage of labor showed no significant difference, with a mean duration of 1.69 ± 0.39 hours in Group I and 1.70 ± 0.46 hours in Group II (p = 0.93).<br/>Notably, the incidence of maternal side effects was significantly lower in the paracetamol group. Dizziness occurred in 17.86% of women in Group I compared to 66.07% in Group II (p = 0.0001). Nausea was reported by 26.79% of women in the paracetamol group, compared to <br/>62.5% in the meperidine group (p = 0.0001), and vomiting was reported by 17.86% of women in Group I versus 53.57% in Group II (p = 0.0008).<br/><br/>Neonatal outcomes were also more favorable in the paracetamol group. The 1-minute Apgar score averaged 7.75 ± 1.01 in Group I, significantly higher than the 6.16 ± 0.95 in Group II (p = 0.0001). The 5-minute Apgar score was 8.18 ± 0.74 in the paracetamol group, compared to 7.16 ± 0.68 in the meperidine group (p = 0.0001). Additionally, the need for neonatal resuscitation was lower in the paracetamol group (12.5% vs. 25%, p = 0.06), though this difference was not statistically significant.<br/>Conclusion: This study suggests that paracetamol is a viable and safer alternative to meperidine for labor pain management in primiparous women. Its use is associated with fewer adverse maternal and neonatal outcomes, making it a preferable choice in clinical settings where minimizing side effects and optimizing maternal and neonatal health are priorities. These findings contribute to evidence-based practices in obstetric pain management, highlighting the need for further research to refine pain management protocols in labor and delivery.<br/><br/><br/>
