Background: Neurological and pulmonary complications following cardiac arrest are significant contributors to morbidity and mortality. Understanding the complex interplay between these complications and their effects on prognosis is crucial.<br/>Objective: To investigate the relationship between pulmonary and neurological complications after cardiac arrest and their impact on patient outcomes.<br/>Methods: A prospective observational cohort study was conducted on 43 patients who experienced in-hospital cardiac arrest at Cairo university hospitals. Patients were assessed for various factors, including CPR cycles, LUS scores, ONSD lactate levels, APACHE II and SOFA scores, CT brain and chest and neurological function. <br/>Results: Lung ultrasound (LUS) scores as well as PaO2/FiO2 ratios on ROSC and day 1 emerged as a strong predictor of mortality and poor neurological outcome (p =0.004, p =0.011, p=0.013, p=0.058, p= 0.008, p=0.001, p= 0.002, p=<0.001) respectively. GCS post ROSC (p=<0.001), abnormal pupil status, unreactive corneal reflex (p=<0.001) and CT Brain findings (p=<0.001, p=0.038) were all correlated to mortality and poor neurological outcome. ONSD was not sufficient to predict mortality or poor outcomes. Other factors associated with mortality and poor neurological outcome included female sex, pre and post-arrest cardiac support (p=0.021, p=0.003), PaO2/FiO2 ratio (p= <0.001), AKI post ROSC(p=<0.001), liver impairment post ROSC (p=<0.001), lactate levels, number of CPR cycles (p=0.012), APACHE II (p=0.004), SOFA score(p=0.012).<br/>Conclusion: Evaluating lactate levels, APACHE II, SOFA, GCS, pupil status, and CT Brain findings can help predict survival and outcomes in post-in-hospital cardiac arrest patients. LUS scores are valuable prognostic tools for assessing lung injury and predicting mortality. |

يجب أن نفهم المضاعفات الرئوية والعصبية التي تحدث للمرضى بعد توقف عضلة القلب وتأثيرها على نسبة الوفيات والحالة العصبية للمرضى حتى نتمكن من تحسين الخدمة المقدمة للمرضى والعلاج بالشكل المناسب.<br/>تهدف هذه الدراسة الى استقصاء وتحديد المضاعفات الرئوية والعصبية التي تحدث بعد السكتة القلبية وتأثيرها على التشخيص والوفيات.<br/>شملت هذه الدراسة 43 مريضًا ممن تعرضوا لتوقف القلب داخل مستشفيات جامعة القاهرة. <br/>أظهرت الدراسة أن ارتفاع نسبة الوفيات وتدهور النتائج العصبية كان مرتبطا:<br/>• بحالة حدقة العين الغير طبيعية في اليوم 0، 1، 3، ورد فعل القرنية الغير تفاعلي في اليوم 0، 1، 3، ونقص في مقياس غلاسكو للغيبوبة في اليوم الأول واليوم التالي واليوم الثالث وتغيرات الأشعة المقطعية للمخ. (قيمة P <0.05)<br/>• بزيادة في التقييم الرقمي للموجات الصوتية للرئة في اليوم الأول واليوم التالي وزيادة بنسبة PaO2/FiO2 عند عودة الدورة الدموية وباليوم التالي. ولم تظهر الأشعة المقطعية على الصدر ارتباطا بنسبة الوفيات او تدهور النتائج العصبية. (قيمة P <0.05)<br/>• بعدد دورات الإنعاش القلبي الرئوي. (قيمة P <0.05)<br/>• بارتفاع اللاكتات عند الإفاقة من الإنعاش القلبي الرئوي واللاكتات بعد 3 ساعات واللاكتات بعد 24 ساعة (قيمة P <0.05). وعلاوة على ذلك، أظهرت قيمًا أقل من قيم تصفية اللاكتات بعد 3 ساعات وتصفية اللاكتات بعد 24 ساعة (قيمة P <0.05).<br/>• بارتفاع التقييم الرقمي من APACHE II and SOFAعند الدخول (القيمة P <0.05).<br/>• بارتفاع متوسط أيام العناية المركزة قبل التوقف بعضلة القلب ومتوسط أيام أعلى في استخدام جهاز التنفس الصناعي قبل توقف عضلة القلب مقارنة بالمرضى ذوي النتائج السيئة (قيمة P <0.05).<br/>• بالسن فوق 60 عامًا، ومرضى الإنتان، ومرضى السكري، والمرضى الذين يعانون من ارتفاع ضغط الدم (قيمة P <0.05).<br/>• بقصور حاد بوظائف الكلى والكبد بعد عودة الدورة الدموية (قيمة P <0.05).<br/>• باحتياج المريض الى ضاغطات الأوعية الدموية أو مقويات لعضلة القلب قبل او بعد توقف بعضلة القلب (قيمة P <0.05).<br/>• لاحظنا في وجود ارتباط بين درجات التقييم الرقمي للموجات فوق الصوتية للرئة في اليوم 0 مع عدد دورات الإنعاش القلبي الرئوي. وعلى العكس من ذلك، كان هناك ارتباط سلبي بينها في اليوم 0 ومقياس غلاسكو للغيبوبة (GCS) في نفس اليوم (قيمة P <0.05). |
