| Summary, etc. | Background & Aim: Head and neck Adenoid cystic carcinoma (ACC) is a rare malignancy of glandular tissues. The clinical course of recurrent and metastatic ACC (R/M ACC) is highly variable with no clear role of the use of systemic chemotherapy. We aimed to analyze the course of the disease and to investigate the impact of systemic chemotherapy regarding radiological response, clinical response with symptom control, overall survival (OS), progression free survival (PFS), tolerance and toxicity in patients with locally advanced and/or metastatic ACC of Head and Neck.<br/>Patients & Methods: A retrospective cohort study included 57 patients with locally advanced and/or metastatic ACC of head and neck presented to the National Cancer Institute – Cairo University (NCI-CU) in the period from January 2015 to December 2021. Patients were divided into 2 groups, one group including 35 symptomatic patients received systemic chemotherapy and the remaining 22 asymptomatic patients were kept under observation.<br/>Results: A total of 57 patients with a median age of 51 years (range: 20-87) and female predominance reaching 61.4% (n=35). In the chemotherapy receipt group (n=35), clinical response with symptom control was observed in 60% of patients (n= 21). The overall response rate (ORR) and disease control rate (DCR) were 25% and 56.3% respectively. There was no survival benefit in the group who received systemic chemotherapy compared to those who didn’t. (P = 0.199 and 0.067 for the OS and PFS respectively).<br/>Conclusions: In our study, systemic chemotherapy showed modest efficacy as regards radiological response and clinical response with symptom control for locally advanced and/or metastatic head and neck ACC patients. |

| Summary, etc. | السرطان الكيسي الغداني بالرأس والرقبة هو ورم خبيث نادر في الأنسجة الغدية. يتميز مسار المرض<br/>المرتجع موضعيا والمنتشر بأنه غير ثابت ومتغير للغاية. وجدنا أن أثر العلاج الكيميائي في المرض<br/>المرتجع موضعيا والمنتشر ليس واضحا بعد لذلك كنا نهدف الى التحقيق في دورالعلاج الكيميائي فيما يتعلق بالاستجابة، السيطرة على أعراض المرض، البقاء على قيد الحياة بشكل عام، البقاء على قيد الحياة دون تطور في نشاط المرض، قدرة تحمل العلاج الكيميائي والمضاعفات السمية في هؤلاء المرضى.<br/>طرق البحث:<br/>هذة الدراسة قد تمت بأثر رجعي وشملت جميع المرضى البالغين الذين تم تشخيصهم بالسرطان الكيسي الغداني بالرأس والرقبة المتقدم موضعيا اوالمنتشر والذين توافدوا على المعهد القومي للأورام- جامعة<br/>القاهرة في الفترة بين يناير 2015 الى ديسمبر2021 وبلغ عددهم 57 مريضا. تم استخراج جميع<br/>المعلومات الاكلينيكية والباثولوجية الخاصة بهؤلاء المرضى من الملفات المسجلة وتم تقسيم المرضى الى<br/>مجموعتين: مجموعة من المرضى لديهم أعراض للمرض تلقوا العلاج الكيميائي ومجموعة لا تحمل<br/>أعراض للمرض ظلت تحت الملاحظة.<br/>النتائج:<br/>شملت الدراسة اجمالي 57 مريض بمتوسط عمر51 عاما )نطاق عمري من 20 الى 87 عام( وكان أغلبهم نساء بلغت نسبتهم %61.4 من اجمالي المرضى.<br/>في مجموعة تلقي العلاج الكيميائي )عدد = (35 كان معدل السيطرة على الأعراض في المرضى %60 )عدد (21 بينما كان معدل الاستجابة الاجمالي ومعدل مكافحة المرض %25 و%56.3 على التوالي. لم<br/>يكن هناك فرق ذو دلالة احصائية فيما يتعلق بالبقاء على قيد الحياة بشكل عام والبقاء على قيد الحياة دون<br/>تطور في نشاط المرض بين مجموعة تلقي العلاج الكيميائي ومجموعة الملاحظة.<br/>الاستنتاج:<br/>وفقا لهذة الدراسة وجد أن العلاج الكيميائي له فعالية مقبولة في مرضى السرطان الكيسي الغداني بالرأس والرقبة المتقدم موضعيا والمنتشر. |
