Summary, etc. Aim of Work: To investigate the effect of addition of intranasal ketamine to midazolam compared to midazolam alone as a premedication on the occurrence of PRAEs.<br/>Patients: Children ASA I&II aged from 3 to 12 years old undergoing AT procedures. <br/>Interventions: The study was a double-blinded randomized controlled trial. 202 Children were randomly assigned to two groups in a 1d1d1 ratio; Group Midazolam (Group M) was to receive sedation by intranasal midazolam before general anesthesia and Group midazolam-ketamine (Group M-K) was to receive intranasal midazolam-ketamine before general anesthesia.<br/>Measurements: Perioperative respiratory adverse events were clustered into two groups: Major as (bronchospasm, laryngospasm, hypoxia [so2 less than 90%], airway obstruction and stridor). Minor as (cough, desaturation from [90%-95%]). Heart rate, mean arterial pressure, and pulse oximeter saturation were recorded. Sedation success rate was recorded using FUNK score, and the post-operative Anxiolysis score was used to assess sedation. Pain was assessed after extubation and at PACU every 30 minutes till discharge using Wong-Baker Pain Scale. Postoperative emergence delirium was assessed using PAED scale.<br/><br/><br/>Results: 199 patients were included in this study, the incidence of PRAEs was lower in midazolam-ketamine group (Group M-K) compared to midazolam group (Group M) alone. The combination group also improved postoperative pain control, higher sedation success rate, and comparable recovery scores, with no significant adverse events related to ketamine administration. <br/>Conclusion: In children undergoing adenotonsillectomy, the preoperative intranasal combination of ketamine and midazolam effectively reduced the incidence of perioperative respiratory adverse events compared to intranasal midazolam alone, without significant adverse events related to ketamine.

Summary, etc. استئصال اللوزتين الغدية هو واحد من أكثر العمليات الجراحية شيوعا بين الأطفال. يمكن أن يعاني الأطفال الذين يخضعون لجراحات التكنولوجيا المساعدة من قلق وضيق كبيرين خلال الفترة المحيطة بالجراحة. من ناحية أخرى ، فإن الأحداث الضائرة التنفسية المحيطة بالجراحة هي المضاعفات الأكثر شيوعا أثناء تخدير الأطفال.<br/>هدفت هذه الدراسة إلى التحقيق في تأثير إضافة الكيتامين داخل الأنف إلى الميدازولام مقارنة بالميدازولام وحده كتخدير على حدوث الأحداث الضائرة التنفسية المحيطة بالجراحة.<br/>تم تسجيل ما مجموعه 199 طفلا في هذه التجربة المعشاة ذات الشواهد. وكانت النتيجة الأولية هي حدوث أي أحداث ضائرة تنفسية في الفترة المحيطة بالجراحة.<br/>أظهرت النتائج أن الجمع بين الكيتامين والميدازولام قلل بشكل كبير من حدوث الاحداث الضئرة التنفسية المحيطة بالجراحة مقارنة بالميدازولام وحده. الأهم من ذلك ، أن تركيبة مجموعة الكيتامين ميدازولام لم تسفر عن آثار ضارة كبيرة تتعلق باستخدام الكيتامين.<br/> في الختام ، يعد التخدير عن طريق الأنف مع تركيبة الكيتامين ميدازولام أكثر فعالية من الميدازولام وحده في الحد من الأحداث الضائرة التنفسية المحيطة بالجراحة لدى الأطفال الذين يخضعون لاستئصال اللوزتين واستئصال اللحمية ، مما يوفر استراتيجية أكثر أمانا وفعالية قبل الجراحة.
