Summary, etc. Cesarean section is one of the most commonly performed surgical procedures in the field of obstetrics. It accounts for more than<br/>one-fourth of all births worldwide. The most common undesirable outcome following cesarean section is postoperative pain. Failure to treat the pain not only causes distress to the mother but also affects mother-baby bonding, which, in turn, affects care and breastfeeding of the newborn. It has been assumed that adequate pain relief will improve the surgical outcome with reduced morbidity, need for hospitalization and convalescence, and there is a common consensus that optimal pain relief is a prerequisite for early postoperative recovery.<br/>Most of the pain experienced by patients after abdominal surgery is derived from the abdominal wall incision. There are three layers of abdominal wall, i.e. the external oblique, internal oblique, and transversus abdominis and their associated fascial sheaths. The muscular wall is innervated by nerve afferents that course through the transversus abdominis neurofascial plane. Blockade of these nerve afferents via abdominal field blocks is limited, and the degree of block achieved could be unpredictable.<br/>This randomized Controlled clinical trial was conducted on 200 pregnant women undergoing cesarean section at maternity hospital of Cairo University. The 200 patients will be divided into 2 equal groups 100 each. started June 2022 and finished June 2023.<br/>The results of this study were:<br/>There was no significant difference between the studied groups regarding, body mass index, parity, indications of section, gestational age and operation duration. But there is significant difference between groups regarding age.<br/>Patients' pain perception (VAS-10) during rest at different follow up times were lower in TAP block group, the differences were<br/><br/> <br/>statistically significant at hours 0.5, 1, 6,18 and 24. Patients' pain perception (VAS-10) during movement at different follow up times were lower in TAP block group, the differences were statistically significant at hours 0.5,1,2 6,18and 24.<br/>Regarding nausea and vomiting score there was high statistically significant difference between both groups as it was lower among TAB block group.<br/>Using mean arterial blood pressure as a tool for follow up showing significant difference between both groups, the results showed higher mean arterial blood pressure in control group at hours 0, 4, 6, 24.<br/>And the main concern for this study is to assess and obtain patient‘s satisfaction which was highly achieved among TAP block group.

Summary, etc. تعتبر الولاده القيصرية من أكثر العمليات الجراحية شيوعاً في مجال النساء والتوليد. وهي تمثل أكثر من ربع جميع الولادات في جميع أنحاء العالم. ويعتبر الألم من المشاكل الأكثر شيوعا بعد الولادة القيصرية وتؤثر علينا الرغبة الملحه لتقليل نسبة الألم بعد الولاده للقيام بهذه الدراسة.<br/> إن الفشل في علاج الألم لا يسبب مشاكل للأم فحسب، بل يؤثر أيضًا على ارتباط الأم بالطفل، مما يؤدي بدوره إلى التأثير على رعاية المولود الجديد وإرضاعه طبيعيًا. من المفترض أن تخفيف الألم بشكل كافٍ سيحسن النتيجة الجراحية مع تقليل معدلات الإصابة بالأمراض ومدة العلاج في المستشفى والنقاهة، وهناك إجماع على أن تخفيف الألم هو شرط أساسي للشفاء المبكر بعد العملية الجراحية.<br/>معظم الألام التي يعاني منها المرضى بعد جراحة البطن أتي من شق جدار البطن. فهناك ثلاث طبقات من جدار البطن، و هم المائلة الخارجية، والمائلة الداخلية، والمستعرضة البطنية وأغماد اللفافة المرتبطة بهم. يتم تغذية الجدار العضلي بواسطة واردات عصبية تتدفق عبر المستوى العصبي الليفي المستعرض البطني. إن الحصار المفروض على هذه الناقلات العصبية عبر كتل المجال البطني محدود، ويمكن أن تكون درجة الكتلة المحققة غير قابلة للتنبؤ بها.<br/>تم إجراء هذه التجربة السريرية العشوائية على 200 امرأة حامل تخضع لولاده قيصرية في مستشفى النساء والتوليد بجامعة القاهرة. حيث تم تقسيم المرضى إلى مجموعتين متساويتين كل منهما 100 مريض. بدأت في يونيو 2022 وانتهت في يونيو 2023.
