Background: Lumbar discectomy and/or laminectomy are most commonly performed under general anesthesia, which can be associated with several perioperative morbidities. It is possible that fewer complications may occur if the procedure is performed under spinal anesthesia. <br/>Objective: The purpose of this study was to evaluate clinical outcomes, compare intra and postoperative parameters, surgeons’ satisfaction, patients’ satisfaction, and cost-effectiveness between general anesthesia (GA) and spinal anesthesia (SA) on patients undergoing lumbar spine surgery. <br/>Methods: A total of 84 patients from February 2022 to February 2023 who underwent either discectomy, laminectomy or lamino-foraminotomy for lumbar disc prolapse or lumbar canal stenosis were included. During the study period, 42 patients were randomly selected for each of the SA or GA groups. The heart rate (HR), mean arterial blood pressure (MABP), blood loss, total anesthetic time, pre and postoperative visual analogue scale (VAS), Oswestry Disability Index (ODI) and short form 36 (SF-36), analgesic requirements, postoperative side effects, hospital stay, patients’ satisfaction, surgeons’ satisfaction, and cost of the procedure were recorded and compared. <br/>Results: In the context of demographic characteristics, baseline HR, or MABP, no significant differences were noted between SA and GA groups, but it has been found that HR and MABP was significantly lower in the SA group after initiation of anesthesia, after initiation of surgery, after completion of surgery, after admission to the post anesthesia care unit (PACU) & before discharge to the general ward. Mean anesthetic time, mean PACU time, mean doses of analgesic requirement, cost of anesthesia, and the surgeons’ satisfaction were significantly lower in the SA Group (P < 0.05). Also, the blood loss, hospital stay, and postoperative side effects were lower in the SA group. <br/>Conclusion: Safety and efficacy of SA in comparison to GA were similar for patients undergoing surgery on the lumbar spine. Notable advantages of SA include shorter anesthesia duration, fewer drug requirements, relative cost-effectiveness, and fewer complications rate. Successful surgery can be performed using either anesthesia type. |

الخلفية: إن عمليات استئصال الانزلاق الغضروفي القطني أو استئصال الصفيحة العظمية القطنية لتوسيع القناة الشوكية القطنية غالبا ما يتم إج ارؤها تحت تأثير التخدير الكلي، والذي يمكن أن يكون مرتبطا بالعديد من المضاعفات المحيطة بالج ارحة مثل الغثيان والقيء وانخماص الرئة وال رشف الرئوي ومدة الإفاقة الطويلة ما بعد التخدير. ربما تكون الج ارحة مصحوبة بمعدل مضاعفات أقل إذا ما تم إج ارؤها تحت تأثير التخدير النخاعي. <br/>مارجعة الأدبيات: إن ج ارحات العمود الفقري القطني هي أكثر ج ارحات العمود الفقري شيوعا. يمكن إج ارء ج ارحات العمود الفقري القطني باستخدام أساليب تخدير مختلفة مثل التخدير العام أو التخدير النخاعي. يعد كل من التخدير العام والتخدير النخاعي أساليب تخدير مناسبة لإج ارء الج ارحة بصورة آمنة. تظل الفائدة الواضحة من إج ارء الج ارحة تحت تأثير التخدير النخاعي غير مثبتة. تم إج ارء العديد من الد ارسات لمقارنة هذين الأسلوبين في التخدير وأظهرت هذه الد ارسات نتائج متباينة. يتم استخدام التخدير العام بصورة أكبر في أغلب الأحيان، لكن التخ دير النخاعي أصبح أكثر شيوعا بصورة مت ازيدة لأنه يتيح للمريض ضبط تموضعه تلقائيا وبالتالي يقلل من المضاعفات المختلفة المرتبطة بالتخدير العام في وضع الاستلقاء والمريض منكب على وجهه. <br/>الغرض من الدارسة: إن الغرض من هذه الد ارسة هو تقييم النتائج السريرية ومقارنة المؤش ارت سواء أثناء أو بعد الج ارحة إلى جانب معدل رضا الج ارحين ومعدل رضا المرضى والفعالية من حيث التكلفة بين التخدير العام والتخدير النخاعي للمرضى الذين يخضعون لعمليات العمود الفقري القطني. <br/>منهجية الدارسة: تتضمن الد ارسة إجمالي عدد 84 مريضا يعانون من الانزلاق الغضروفي القطني أو ضيق القناة الشوكية القطنية وخضعوا لإج ارء عمليات ج ارحية لاستئصال الانزلاق الغضروفي القطني، أو استئصال الصفيحة العظمية القطنية لتوسيع القناة الشوكية القطنية، أو استئصال الصفيحة العظمية القطنية وتوسيع مخارج جذور الأعصاب وذلك في الفترة من فب ارير 2022 م وحتى فب ارير 2023 م. وخلال مدة الد ارسة خضع 42 مريضا للقيام بإج ارء هذه التدخلات الج ارحية تحت تأثير التخدير النخاعي بينما خضع 42 مريضا آخرون للقيام بهذه التدخلات الج ارحية تحت تأ ثير التخدير العام حيث تم اختيار المرضى لأي من المجموعتين بصورة عشوائية. ولقد تم تسجيل كل من معدل ضربات القلب، ومعدل ضغط الدم الشرياني الوسطي، ومعدل فقد الدم أثناء الج ارحة، ووقت التخدير الإجمالي، ومقياس النظير البصري ومؤشر أوسويستري للإعاقة والاستمارة القصيرة |
