| Summary, etc. | Background: Acute heart failure (AHF) is a prevalent and life-threatening condition that is associated with high morbidity and mortality, particularly among adults. Timely and accurate diagnosis and prognosis are essential for effective management. MicroRNAs (miRNAs) have emerged as promising biomarkers in various cardiovascular diseases. Specifically, miR-302b-3p and miR-103 have shown potential as biomarkers in heart failure. However, their role in AHF diagnosis and prognosis, especially in Egyptian adults, remains underexplored. Additionally, the relationship between these miRNAs and NT-Pro BNP, a key biomarker for heart failure, warrants investigation.<br/>Objectives: This study aims to investigate the diagnostic and prognostic potential of circulating miR-302b-3p and miR-103 in AHF among Egyptian adults. It also aims to explore the relationship between these miRNAs and NT-Pro BNP levels in AHF patients.<br/>Subjects and Methods: The study involved 95 participants, consisting of 45 Egyptian adults diagnosed with AHF and 50 age- and gender-matched healthy controls. Circulating levels of miR-302b-3p and miR-103 were measured using quantitative PCR, while NT-Pro BNP levels were determined via enzyme-linked immunosorbent assay (ELISA). Clinical and demographic data were collected to assess the association of miRNAs with the severity and prognosis of AHF.<br/>Results: miR-302b-3p was significantly decreased in AHF patients compared to healthy controls (p < 0.05). The levels of this miRNA exhibited a strong negative correlation with NT-Pro BNP levels (r = 0.78, p P value 0.013). on the other hand, no significant difference was detected in fold change in miR-103 between the cases and controls. Decreased miR-302b-3p and elevated NT-ProBNP levels were associated with poorer prognosis, including higher rates of hospitalization and 1-year mortality in AHF patients.(P<0.05)<br/>Conclusion: Circulating miR-302b-3p represent promising biomarker for the diagnosis and prognosis of AHF in Egyptian adults. Their strong correlation with NT- Pro BNP further supports its potential role in clinical practice. These findings suggest that this miRNA may aid in the early detection and stratification of AHF, ultimately improving patient management. |
