| Summary, etc. | Background: Sciatica is a medical term that describes a sensation of burning, tingling and numbness experienced in the buttock, thigh, leg as well as foot. Objective: The purpose of this study was to compare between the effect of neurodynamics mobilization and Mulligan mobilization (Sustained Natural Apophyseal Glides) on pain pressure threshold, hip flexion range of motion (ROM) of straight leg raise as well as functional abilities in patients suffering from chronic unilateral discogenic sciatica. Methods: Sixty-six male lumber radiculopathy patients were randomized into three equivalent groups. Control group (G1) was treated by conventional physical therapy treatment that includes therapeutic strengthening exercises (back extension exercise and prone hip extension exercise) and electrotherapy (transcutaneous electrical nerve stimulation and ultrasound). Two Study groups (G2 and G3), study group (G2) was treated by Mulligan SNAG mobilization technique in addition to conventional physical therapy program. Study group (G3) treated by neurodynamic mobilization for sciatic nerve from different positions (sliding and tensioning techniques) in addition to conventional physical therapy program. The duration of each session was one hour, three times per weeks for four weeks. Pre and post treatment evaluation was done for all variables in all groups. Pain pressure threshold was measured by pain pressure algometer, sciatica symptoms intensity was assessed by Sciatica Bothersomeness Index and, hip flexion range of motion of straight leg raise was measured by inclinometer and functional abilities were assessed by Roland‐ Morris Disability Questionnaire. Results: There was no statistically significant differences among the three groups in pretreatment mean values of all measured variables. While post treatment, a statistically significant difference was observed in the study group (G3) compared to the control group (G1) and the study group (G2), in the following variables (hip ROM , pain pressure threshold and functional abilities) with a p-value of less than 0.05. Conclusion: This study showed both mulligan SNAG mobilization and neurodynamic mobilization add a valuable effect in improving pain pressure threshold, hip flexion ROM of straight leg raise and functional abilities with more superiorly to neurodynamic mobilization. |

| Summary, etc. | الخلفية: عرق النْسّا هو مصطلح طبي يصف إحساسًا بالحرقان والوخز والخدر الذي يُشعر به في الأرداف والفخذ والساق وكذلك القدم.<br/>الهدف: تهدف هذه الدراسة إلى مقارنة تأثير التحريك العصبيي الديناميكي (تقنيات الانزلاق والشد) ضد موليجان على تحمل ألم الضغط ، ومدى انثناء الورك أثناء رفع الساق المستقيمة، بالإضافة إلى القدرات الوظيفية لدى المرضى الذين يعانون من عرق النْسًا المزمن الناتج عن انزلاق غضروفي أحادي الجانب.<br/>الطرق: تم اختيار 66 مريضًا من الذكور الذين يعانون من عرق النْسّا المزمن الناتج عن انزلاق غضروفي أحادي الجانب.وتوزيعهم بشكل عشوائي على ثلاث مجموعات متساوية. تلقت المجموعة الضابطة (المجموعة الاولي) علاجًا طبيعيًا تقليديًا شمل تمارين تقوية علاجية (مثل تمارين تمديد الظهر وانثناء الورك في وضعية الانبطاح) والعلاج الكهربائي (التحفيز الكهربائي للعصب عبر الجلد والموجات فوق الصوتية).و تلقت المجموعة التجريبية الأولى (المجموعة الثانية) تقنية موليجان بالإضافة إلى برنامج العلاج الطبيعي التقليدي، بينما خضعت المجموعة (المجموعة الثالثة) لتقنية التحريك العصبيي الديناميكي لعصب الورك باستخدام تقنيات الانزلاق والشد، بالإضافة إلى برنامج العلاج الطبيعي التقليدي. استمرت كل جلسة لمدة ساعة واحدة، ثلاث مرات في الأسبوع على مدى أربعة أسابيع. تم تقييم جميع المتغيرات قبل العلاج وبعده في جميع المجموعات. تم قياس تحمل ألم الضغط باستخدام مقياس ضغط الألم، وشدة أعراض عرق النْسّا باستخدام مؤشر إزعاج عرق النْسّا، ومدى انثناء الورك أثناء رفع الساق المستقيمة باستخدام مقياس الميل ، كما تم تقييم القدرات الوظيفية باستخدام استبيان رولاند موريس للإعاقة.<br/>النتائج: لم تكن هناك فروق ذات دلالة إحصائية بين المجموعات في القيم المتوسطة للمتغيرات المقاسة قبل العلاج ، لوحظت فروق ذات دلالة إحصائية في (المجموعة الثالثة) مقارنة بالمجموعة الضابطة (المجموعة الاولي) و(المجموعة الثانية) في المتغيرات التالية: مدى انثناء الورك و تحمل ضغط الألم والقدرات الوظيفية بعد العلاج.<br/> الاستنتاج: أظهرت الدراسة أن كلاً من تقنيتي موليجان و التحريك العصبيي الديناميكي تضيفان قيمة فعالة في تحسين تحمل ضغط الألم، ومدى انثناء الورك أثناء رفع الساق المستقيمة، والقدرات الوظيفية، مع تفوق أكبر لتقنية التحريك العصبيي الديناميكي. |
