Background: The aim of this work was to compare the Laryseal TM Pro and the Ambu AuraGain in terms of airway seal pressure and success rate of insertion in ASAI & II adult patients undergoing elective ophthalmic operations.<br/>Methods: 54 patients (18-55 years) of both gender with American society of Anesthesiologists (ASA) physical status I & II, Ganzori Score less than 3 and scheduled for elective ophthalmology operations were enrolled in this study.<br/>However, patients with coagulation disorder, upper respiratory tract infections, obstructive sleep apnea (OSA), full stomach (trauma, morbid obesity BMI> 40Kg/m2, pregnancy, history of gastric regurgitation and hiatus hernia), active chest condition (pneumonia, acute exacerbation of bronchial asthma, interstitial lung disease and COPD) or cardiac conditions (angina, MI, decompensated heart failure, and arrhythmia) were excluded from the study.<br/>The primary outcome was measuring the Oropharyngeal Seal Pressure in CmH2O after 5min of proper placement of the EAD. Secondary outcomes were Insertion time of EAD in seconds, Success rate of first attempts and second attempts, Total number of insertions attempts of EAD, Ease of EAD Insertion Score, Oropharyngeal Seal pressure 15, 30, and 60 min after proper insertion, The laryngeal view grade based on Brimacombe and Berry Score by flexible bronchoscope, and Postoperative complications as such as (dysphagia, sore throat, and blood streaked mucous)<br/> <br/>Results: Oropharyngeal seal pressure was significantly higher in Group L than in Group A at 5 min after proper insertion, 15, 30, and 60 min, Insertion time of EAD was significantly higher in Group A than in Group<br/>L. Regarding manipulation needed for insertion, neck extension was needed in only one patient (3.7%) in Group L, while it was needed in 26(96.2%) patients in Group A. Manipulation needed for insertion (neck extension) was significantly lower in group L than Group A. Hemodynamic measurements were insignificantly different between both groups. Laryngeal View Grade 1 (B&B Score 4) was seen in 16 (59.3%) patients in Group L and 12 (44.4%) patients in Group A. This was statistically insignificant. Postoperative complications were insignificantly different between both groups.<br/>Conclusion: In conclusion, we found that The Laryseal Pro TM seemed to provide a significantly better oropharyngeal seal pressure, an easier and shorter insertion time, compared to Ambu AuraGain™ and can be considered as an option for ventilation in adult patients undergoing elective ophthalmic operations under general anesthesia. |

المقدمة:<br/>تم اختراع أجهزة المجرى الهوائي خارج الحنجرة في المجال الطبي لتسهيل التنفس والتنبيب الرغامي خاصة في حالة صعوبة إدارة المجرى الهوائي.وتوصي بها توجيهات المجرى الهوائي الصعبة كخطة إنقاذ ثانية في حالة صعوبة التنفس أو التنبيب.<br/>يعتبر جهاز أمبو أورا جين (Ambu Aura Gain) جهازًا من الجيل الثاني لأجهزة المجرى الهوائي خارج الحنجرة، ويتميز بنفق ذي قطر داخلي واسع يُستخدم كقناة لأنبوب القصبة الهوائية.يساعد وجود وصلة للمعدة من خلال منفذ ثانٍ على تصريف عصارة المعدة وشفط العصارة المعدية.يتميز جهاز أمبو أورا جين بمنحنى تعليمي سريع للإدخال.يأتي الجهاز بأحجام مختلفة ليناسب مختلف الفئات العمرية وأوزان الجسم المختلفة.<br/>يعتبر جهاز لاريسيل برو (Laryseal TM Pro) المخترع حديثًا جهازًا من الجيل الثاني لأجهزة المجرى الهوائي خارج الحنجرة المستخدمة لإدارة المجرى الهوائي بأمان وفعالية.يحتوي تصميم الجهاز على كفة متناظرة، ونفق أنبوبي ثابت على شكل حرف C يوجد بداخله منحدر صغير لتسهيل وتوجيه إدخال الأنبوب الرغامي.وهو موجود في المجال الطبي بأحجام مختلفة لتناسب جميع المرضى.<br/>يُعرف ضغط تسرب البلعوم الفموي بأنه الضغط الذي يكسر الغطاء بين جهاز المجرى الهوائي خارج الحنجرة والغشاء المخاطي المحيط به؛ وغالبًا ما يُستخدم كمقياس بديل لوضع الجهاز الصحيح.كلما ارتفع ضغط تسرب البلعوم الفموي، كان ذلك أفضل خاصة عند استخدام جهاز المجرى الهوائي خارج الحنجرة للتنفس الاصطناعي.<br/>يمكن استخدام جهاز المجرى الهوائي خارج الحنجرة في جراحات العيون بهدف تجنب زيادة ضغط العين التي قد تحدث أثناء التنبيب الرغامي. |
