| Summary, etc. | Background: Stroke is one of the leading causes of death and disability in developing countries. Aim: Is to examine the effect of a designed nursing intervention protocol on neurological outcomes among acute stroke patients. Five research hypotheses were formulated. Design: a quasi-experimental research design was utilized. Sample & setting: 60 matched adult male and female acute ischemic stroke patients in the stroke in El-Kaser Al-ainy hospital. Tools: National Institutes of Health Stroke Scale, modified Rankin scale, Gugging Test, wells' score, oral health assessment tool, Level of Mobility and the Hemodynamic data Sheet were used to collect data. Patients were assessed three times; pre-intervention, post interventions and pre-discharge to examine the stated research hypotheses. Results: Hypothesis one was supported showing a statistical difference between the study and the control groups as regards to the severity of stroke. Regarding degree of disability, hypothesis two was supported that found a statistical difference between both groups. In addition, hypothesis three wasn’t supported in relation to hemodynamic parameters including temperature, pulse, blood pressure and random blood sugar. Concerning risk for aspiration, a significant statistical difference was found between both groups so, hypothesis four was supported. Hypothesis five was supported, as study group showing fewer complications than control group regarding risk of developing deep venous thrombosis, mobility level and oral health status. Conclusion: It can be concluded that neurological outcomes of acute ischemic stroke patients were improved when applying a designed nursing intervention protocol through reducing severity of stroke, level of disability and a risk of aspiration. |

| Summary, etc. | تعد السكتة الدماغية هي السبب الثاني الرئيسي للوفاة و الثالث في الإعاقة على مستوى العالم. يختلف معدل حدوث السكتة الدماغية و الوفيات المرتبطة بها بين الدول و المناطق الجغرافية و المجموعات العرقية. تعد السكتة الدماغية الإقفارية واحدة من أهم أسباب الوفاة و الإعاقة العصبية الوعائية، حيث تتأثر بعدد من عوامل الخطر مثل التدخين و مؤشر كتلة الجسم المرتفع (BMI)، و التي تعتبر أيضاً حاسمة في الوقاية من الأمراض القلبية و الدماغية الوعائية الأخرى.<br/>تتميز السكتة الدماغية بفقدان مفاجئ للوظيفة، مع فقدان عصبي محدد حسب الجزء المتأثر من الدماغ. العلامة الأكثر وضوحاً للسكتة الدماغية هي شلل نصفي يمكن أن يؤثر على طرف واحد أو كلا الطرفين، و قد يتطور في أشد حالاته إلى شلل نصفي كامل. تشمل علامات السكتة الدماغية الأخرى فقدان اللغة، تشوش أو فقدان الرؤية، فقدان التوازن أو التنسيق، و فقدان الوعي. قد تستمر الأعراض العصبية لعدة أشهر أو سنوات، و يتطلب العديد من المرضى إعادة تأهيل عصبي مكثف، و حتى بعد ذلك قد لا تزول الأعراض تماماً.<br/>يلعب الممرضون دوراً مهماً جداً في رعاية و توجيه مرضى السكتة الدماغية. خلال المرحلة الحادة، يراقب الممرضون حدوث مضاعفات مثل التهاب الرئة، القروح الناتجة عن الاستلقاء، التهابات المسالك البولية، تجلط الدم، الهذيان، الاكتئاب، درجة وعي المريض، إصابة الجانب المتأثر بسبب الإهمال، وضعية غير ملائمة، و عدم كفاية تناول الطعام أو السوائل. يطبق الممرضون التدخلات الوقائية في وحدة رعاية السكتة الدماغية وفقاً لبروتوكولات المستشفى لتقليل مخاطر هذه المضاعفات. |
