Summary, etc. Background: Epilepsy is the 2nd most common neurological disorder with 52 million patients suffering from its morbidities worldwide. Epilepsy is a complex condition with a multitude of causes that necessitate extensive laboratory, neurophysiologic and imaging investigations. Most of epileptic patients (about 80%) live in low-income countries with poor health sector resources. Approximately 33% of epileptic patients are drug resistant and referring to surgical ablation of epileptic focus can be the treatment of choice. Structural MRI studies fail to detect an ablatable/resectable focus in about 40% of those patients and standard electro-encephalogram procedure (EEG) eventually fails to localize or capture this epileptogenic focus. Non-contrast assessment of cerebral blood flow using 3D-pseduo continuous arterial spin labelling (3D-pCASL) was reported as a highly cost effective and safe procedure with diagnostic capabilities comparable to both FDG18-PET and H2O15-PET studies in localizing epileptic foci and mapping out epileptic networks in an ultra-short examination time and associated ultra-high structural resolution of simultaneously acquired MRI study compared to longer EEG examination time and poor spatial resolution.<br/>Objectives and aim of work: To assess the sensitivity of inter-ictal 3D-pCASL in detection and lateralization of epileptic foci in comparison to the widely available, routinely performed, standard EEG procedure.<br/>Methods: The study was carried out on 60 epileptic patients. After clinical evaluation and performing EEG, structural MRI as well as 3D pCASL were carried out to detect any structural abnormality and assess mesiotemporal lobe CBF values respectively. Pearson correlation test was used to assess correlations between the EEG and 3D PCASL results to calculate the concordance between them in addition to comparing their sensitivity. ROC analysis was conducted to assess the cut-off percentage difference between CBF values of both mesiotemporal lobes at which lateralization of the epileptogenic focus can be determined. Level of agreement between diagnostic tests was conducted using Kappa test. <br/>Results: pCASL has shown to be a reliable method for lateralizing epileptogenic foci when compared to routine EEG, being more sensitive and more accurate in terms of localization.<br/>Conclusion: MRI 3D PCASL is a promising noninvasive modality in evaluation of the epileptogenic focus lateralization.
