Summary, etc.	Background: Deep neck space infections (DNIs) are infections involving the fascial planes and spaces of the head and neck. These infections are relatively common otolaryngologic emergencies and can result in significant morbidity with potential mortality. Surgical incision and drainage are considered the mainstay of treatment for DNIs. Image-guided, minimally invasive procedures are alternative methods to the surgical treatment of these infections. Here we compare the standard open surgical drainage with the evolving technique of ultrasound-guided drainage of DNIs with abscess formation with regards to recurrence rate, scar formation, pain & length of hospital stay outcomes.<br/>Study design: Fifty-two patients were randomly allocated using a computer-generated table into two groups. Open surgical drainage was used for group (A) and an ultrasound-guided drainage approach was adopted for group (B). All patients were subjected to pre-operative CT neck with I.V. contrast.<br/>Results: There was a statistically significant difference in the length of hospital stay between the two groups with P-value = 0.042 with the ultra- sound guided drainage group showing better results. The pain and scar formation were all significantly better in the ultrasound-guided drainage group.<br/>Conclusion: It was concluded that ultrasound-guided percutaneous drainage was an effective treatment for well-defined unilocular abscesses in the head and neck.

Summary, etc.	عدوى الفراغات العميقة في الرقبة تشكل حالة تصيب المستويات والفراغات اللفافية في منطقة الرأس والرقبة. تعتبر هذه العدوى شائعة نسبيًا في سياقات الطوارئ ضمن تخصص طب الأنف والأذن والحنجرة، وقد تؤدي إلى مضاعفات خطيرة تهدد الحياة. تتجلى العدوى العميقة في الرقبة غالبًا في صورة خراجات أو التهابات في الأنسجة الخلوية الناتجة عن التهديدات الممكنة في الفراغات اللفافية للرقبة، مثل الفراغات تحت الفك السفلي، وحول البلعوم، وخلف البلعوم. يتطور التهاب الفراغات العميقة في الرقبة بسرعة، الأمر الذي قد يتسبب في مضاعفات تهدد الحياة، بما في ذلك التهاب اللفافة الناخر، وانسداد مجرى الهواء، والتهاب الرئة، وتسمم الدم، والتهاب المنصف الناخر الهابط، والتخثر المنتشر داخل الأوعية الدموية، وتآكل الأوعية الرئيسية، وخثار الوريد الوداجي، والتهاب التامور، وتكون صديد بالغشاء الجنبي. يُعَدُّ التشخيص السريري والشعاعي المبكر، والعلاج بالمضادات الحيوية الوريدية الفعالة، والخطوات الجراحية السريعة لتصريف القيح والحفاظ على مجرى الهواء من أهم المقومات لتحقيق نتائج سريرية مثلى. تُعتبر عملية الشق الجراحي والتصريف الركيزة الأساسية لعلاج عدوى الفراغات العميقة في الرقبة. وقد يؤدي التصريف الجراحي الأعمى نسبيًا للخراج إلى تضرر مفرط للأنسجة، وشقوق واسعة النطاق غير ضرورية، وزيادة في زمن العملية، وألم، فضلاً عن الفشل في تحديد موقع تجويف الخراج وإخلائه. تشكّل الإجراءات الجراحية القليلة التوغل الموجهة بالتصوير بدائل علاجية فعّالة لهذه الالتهابات. تهدف هذه الدراسة إلى مقارنة التصريف الجراحي المفتوح القياسي مع التقنية المتطورة لتصريف عدوى الفراغات العميقة في الرقبة الموجهة بالموجات فوق الصوتية في حالة تكوين الخراج.<br/>تمت الدراسة على 52 مريضًا من وحدة الطوارئ لطب الأنف والأذن والحنجرة في مستشفى قصر العيني، حيث كان جميعهم يعانون من عدوى الفراغات العميقة في الرقبة المصحوبة بتكوين خراج. استوفى جميع المرضى بروتوكول التقييم قبل الجراحة، والذي شمل أخذ التاريخ المرضي، والفحص السريري للفم والرأس والرقبة، والتصوير باستخدام الأشعة المقطعية بالصبغة علي الرقبة. تم تقسيم المرضى إلى مجموعتين: المجموعة (أ) خضعت لتصريف جراحي مفتوح، بينما خضعت المجموعة (ب) لتصريف موجه بالموجات فوق الصوتية. أظهرت النتائج وجود اختلاف ذو دلالة إحصائية في مدة الإقامة في المستشفى بين المجموعتين، حيث أظهرت مجموعة التصريف الموجه بالموجات فوق الصوتية نتائج أفضل. كما كانت معايير الألم وتكون الندبة بشكل أفضل وبفارق ملحوظ إحصائيا في مجموعة التصريف الموجه بالموجات فوق الصوتية.
