Summary, etc.	Aim: The aim of this study was to evaluate the dimensional alveolar bone changes in immediate post extraction implant placement with loading compared to immediate post extraction implant placement without loading in the maxillary premolar area Patients and methods: Thirty Patients were randomly assigned into two groups. Group A: Immediate Implants with loading, Group B: Immediate Implants without loading, Atraumatic extraction is done, Implant insertion is performed. The gap between the implant and socket wall is filled with bone graft if needed. Height of alveolar bone crest, Alveolar bone width, Implant stability WERE assessed. CBCT images are used for evaluation of alveolar bone width around all implant sides all over. At 0. 3, 6 months postoperative by using superimposition and subtraction method. Results: At 3 months, 6 months, Bone height Change, Bone width Change stability Change of both groups, the difference was non-significant Conclusion: Based on the results of this study, immediate loading of dental implant, under certain conditions, is a successful treatment process. Immediate loading is effective in reducing treatment time. Immediate loading represents a valid alternative to the traditional delayed loading rehabilitation.

Summary, etc.	المرضى<br/>تم اختيار ثلاثين مريضًا من العيادة الخارجية لقسم طب الفم وعلاج اللثة بجامعة القاهرة. تم إجراء تصوير الأشعة المقطعية (CBCT) لكل مريض قبل الجراحة. تم تقسيم المرضى عشوائيًا إلى مجموعتين. <br/>•المجموعة أ: زراعة الأسنان الفورية مع التحميل <br/>•المجموعة ب: زراعة الأسنان الفورية بدون التحميل<br/>الإجراء الجراحي<br/>تم إجراء خلع غير مؤلم باستخدام المساعد الجراحي. ثم تم إزالة الحطام المتبقي من تجويف الخلع باستخدام اداه جراحية أو مكحت، وتم فحص التجويف للتأكد من سلامة العظام السنخية. تم إدخال الغرسة وفقًا لتعليمات الشركة المصنعة. تم وضع الغرسة في العظام السنخية وبمسافة 3 مم إلى 4 مم أبعد من هامش اللثة لتحسين التجميل. بعد وضع الغرسة، تم تسجيل المسافة من الغرسة إلى الموضع الرأسي للغرسة وإلى السطح الداخلي العظام السنخية. تم ملء الفجوة بين الغرسة وجدار التجويف بالعظم إذا لزم الأمر. ثم تم تخصيص هذه الغرسات لإحدى المجموعتين: غرسة فورية مع التحميل (مجموعة الاختبار) أو بدون التحميل (مجموعة التحكم) باستخدام طريقة التوزيع العشوائي البسيطة.<br/>التقييم<br/>1.ارتفاع قمة العظم السنخي<br/>2.عرض العظم السنخي<br/>3.ثبات الغرسة<br/>تم استخدام صور CBCT لتقييم عرض العظم السنخي حول جميع جوانب الغرسة على فترات 0، 3، 6 أشهر بعد الجراحة باستخدام طريقة التراكب والطرح.<br/>عند 3 أشهر، كان تغير ارتفاع العظم لمجموعة I (6.30±8.63) ومجموعة II (3.32±6.20) الفارق لم يكن ذو دلالة إحصائية (p=0.343).<br/>عند 6 أشهر، كان تغير ارتفاع العظم لمجموعة I (9.26±9.51) ومجموعة II (8.00±4.61) الفارق لم يكن ذو دلالة إحصائية (p=0.684).<br/>عند 3 أشهر، كان تغير عرض العظم لمجموعة I (2.72±2.34) ومجموعة II (2.14±6.99) الفارق لم يكن ذو دلالة إحصائية (p=0.787).<br/>عند 6 أشهر، كان تغير عرض العظم لمجموعة I (4.54±2.94) ومجموعة II (4.78±5.72) الفارق لم يكن ذو دلالة إحصائية (p=0.898).<br/>1.عند 3 أشهر، كان تغير ثبات الغرسة لمجموعة I (8.99±8.77) ومجموعة II (14.08±7.23) الفارق لم يكن ذو دلالة إحصائية (p=0.135).<br/>2.عند 6 أشهر، كان تغير ثبات الغرسة لمجموعة I (20.99±8.94) ومجموعة II (25.30±7.11) الفارق لم يكن ذو دلالة إحصائية (p=0.205).<br/>تم إجراء اختبار t المستقل لتقييم الرضا بين المجموعتين. مجموعة I كانت (2.1667±0.71774) ومجموعة II كانت (2.1667±0.83485) الفارق لم يكن ذو دلالة إحصائية (p=1.000).<br/>الاستنتاج والتوصيات<br/>1.بناءً على نتائج هذه الدراسة، فإن التحميل الفوري للغرسات السنية، تحت ظروف معينة، هو عملية علاجية ناجحة.<br/>2.التحميل الفوري فعال في تقليل وقت العلاج.<br/>3.يمثل التحميل الفوري بديلاً صالحًا لإعادة التأهيل التقليدي بالتأخير<br/>
