Summary, etc. Background: The pathophysiology of the development of PAH in CAEBV is unclear. It may be related to lymphocytic infiltration and the resultant damage to the pulmonary vascular endothelium caused by EBV infection or may occur secondary to the vascular damage caused by inflammatory reactions induced by EBV infection.<br/>Aim of work: This study’s aim was to evaluate the role of EBV infection in PAH (Idiopathic and connective tissue disease (CTD) patient). The study was done to find CAEBV infection maybe a cause or aggravating factor in pulmonary arterial hypertension patients. <br/>Patients and methods: The 42 PAH patients were selected after doing all routine investigations for pulmonary hypertension and exclusion of other causes of PAH and underwent routine right heart catheterization. The balloon tip of the Swan Ganz Catheter was transferred in Trypsin-EDTA that was added to the transport solution to the clinical pathology lab where tissue culture took place. 10 ml of blood was withdrawn from the patients and the 36 healthy controls as well for blood EBV PCR and Serology testing (IgG and IgM).<br/>Results: There was significant increase in cases than controls regarding the presence of EBV Serum IgM. There was also significant increase in cases than controls regarding the presence of CAEBV infection. Regarding the hemodynamics in the right heart catheter (RHC), there was significant association between degree of dysplasia in endothelial cell culture and mean right atrial pressure (mRAP), pulmonary vascular resistance (PVR) and pulmonary vascular resistance index (PVRI) of idiopathic cases. Otherwise, the results showed no statical significance. <br/>Conclusion: It was concluded that there was significant increase in cases than controls regarding the presence of CAEBV infection. There was also a significant increase in cases than controls regarding the presence EBV Serum IgM. Never the less, there was no significant difference between cases and control regarding the median (IQR) of EBV Serum IgG, IgM and Blood PCR. Nor was there a significant statistical result regarding the presence of the EBV in the endothelial cells of the pulmonary artery with the presence of dysplasia. Given the previous results and reviewing the literature, we cannot establish a direct role for CAEBV in patients with PAH

Summary, etc. ارتفاع ضغط الشريان الرئوي هو اضطراب مزمن ومتقدم يتميز باعتلال الأوعية الدموية التكاثري في الشرايين الرئوية، مما يؤدي إلى تكاثر الخلايا البطانية والعضلات الملساء واختلال وظائفها، والالتهابات والجلطات. تزيد هذه التغييرات من مقاومة الأوعية الدموية الرئوية وضغط الشريان الرئوي اللاحق.<br/>عادةً ما يتم تعريف مرض فيروس إبشتاين بار النشط المزمن (CAEBV) بأنه مرض مزمن يستمر لمدة 6 أشهر على الأقل، مع زيادة مستوى فيروس إبشتاين بار في الأنسجة أو الدم. يرتبط عدوى فيروس إبشتاين بار بمجموعة متنوعة من أمراض المناعة الذاتية و الاورام.<br/>هناك آليتان مرضيتان محتملتان يتسبب فيهما فيروس إبشتاين بار في ارتفاع ضغط الشريان الرئوي، إحداهما أن فيروس إبشتاين بار يسبب التهاب الشرايين الرئوي مما يؤدي إلى إعادة تشكيل هيكل الشريان الرئوي، والأخرى هي أن العدوى المزمنة بفيروس إبشتاين بار تسبب مستويات عالية من السيتوكينات الالتهابية التي تسبب تغيرًا التهابيًا في الشريان الرئوي.<br/>هدفت دراستنا إلى تقييم دور فيروس إبشتاين بار النشط المزمن في مرضى ارتفاع ضغط الشريان الرئوي. سواء كان CAEBV سببًا أو ارتباطًا مشتركًا أو عاملًا مفاقمًا.<br/>أجريت دراستنا في مستشفيات قصر العيني، قسم أمراض الصدر، وحدة أمراض الأوعية الدموية الرئوية، وشملت 42 مريضًا بارتفاع ضغط الشريان الرئوي و36 من الأشخاص الأصحاء العينة الضابطة، الذين تطابقوا في العمر والجنس.<br/>تتلخص الدراسة إلى وجود زيادة كبيرة في الحالات مقارنة بالضوابط فيما يتعلق بوجود عدوى CAEBV.لم يكن هناك فرق ذو دلالة احصائية بين الحالات والضوابط فيما يتعلق بالمدى الربيعي (IQR) لـ الاجسام المضادة المناعية من نوع ال IgG و IgM و تحاليل تفاعل البوليميرازالمتسلسلPCR في الدم.
