| Summary, etc. | Background: An estimated 18.6 million diabetics worldwide get foot ulcers every year and around 20% of patients suffering from DFU will undergo lower extremity amputation.<br/>For limb salvage, bone-culture-specific antibiotics must be started as soon as osteomyelitis in diabetic foot is diagnosed. As a result, MRI and other radiologic modalities are utilized to evaluate the involvement of bones in diabetic foot ulcers. Diffusion weighted imaging is evaluated for whether it adds diagnostic value to conventional MRI in diagnosis of diabetic foot infections.<br/>Methods: 30 patients with clinically suspected osteomyelitis underwent conventional MRI with added diffusion sequence. Three readers evaluated the films, once without the diffusion films and once after 4 weeks with them included. The presence of osteomyelitis or soft tissue abscess was assessed and confidence level recorded before and after the addition of DWI images and ADC map.<br/>Results: Mean ADC for bone in osteomyelitis positive cases was 1.2 ± 0.3 x 10-3 mm/s while osteomyelitis negative cases had bone mean ADC of 0.4 ± 0.2 x 10-3 mm/s. ROC curve analysis to explore the discriminant analysis of Mean ADC of bone in differentiation of OM showed AUC of 0.9 and p-value < 0.0001 for cut off value of ADC >0.7 x 10-3 mm/s with sensitivity of 92.9% and specificity 93.7%.<br/>Mean ADC for soft tissue abscess positive cases was 0.6 ± 0.2 x 10-3 mm/s while soft tissue abscess negative cases had mean ADC of 2 ± 0.5 x 10-3 mm/s. ROC curve analysis to explore the discriminant analysis of Mean ADC of soft tissue in differentiation of soft tissue abscess showed AUC of 0.9 and p-value < 0.0001 for a cut off ADC < 1.3 10 -3 mm2/s with sensitivity of 100% and specificity 93.7%. <br/>Pooled confidence in diagnosis of osteomyelitis using cMRI was 3.8 ± 0.5 which increased to 4.7 ± 0.5 with the addition of DWI with p-value < 0.001. Pooled confidence in diagnosis of soft tissue abscess using cMRI was 4.1 ± 0.6 which increased to 5 ± 0 with the addition of DWI with p-value < 0.00. <br/>Conclusion: Diffusion weighted images add to the confidence of readers with great inter-reader agreement; ADC value calculation can differentiate between healthy and abnormal bone marrow with high level of sensitivity and specificity. |

| Summary, etc. | تشكل تقرحات القدم السكرية واحدة من المضاعفات الرئيسية لدى مرضى السكري. قد تقتصر التهابات القدم السكرية على الأنسجة الرخوة أو تشمل عظام القدم التي تسبب التهاب عظم القدم السكري ، والذي قد يبلغ ذروته في بتر الأطراف السفلية في ثلثي المرضى. لذلك ، يعد التشخيص في الوقت المناسب لالتهاب العظم والنقي في القدم أمرا حيويا لبدء العلاج الطبي أو الجراحي المناسب لإعطاء الأولوية لإنقاذ الأطراف. <br/>حاليا ، يستخدم التصوير بالرنين المغناطيسي على نطاق واسع كطريقة تشخيصية مفضلة للتشخيص المبكر لالتهاب العظم والنقي لأنه يعطي تفاصيل شاملة عن طبيعة ومدى إصابة كل من الأنسجة العظمية والرخوة. يتمتع التصوير بالرنين المغناطيسي بحساسية جيدة للكشف عن التهاب العظم والنقي (90٪) ، ولكن قد تتراوح الخصوصية من 70٪ إلى 80٪ أو حتى أقل. يعد استخدام صبغة الجادولينيوم مفيدا لتحديد تجمع السوائل / الخراجات ، والمسالك الجيبية ، والمناطق الغير حية.<br/>في هذه الدراسة، قمنا بمقارنة ثقة قمنا بتصوير ثلاثين مريض قدم سكري بالرنين المغناطيسي بالصبغة و أضفنا خاصية الانتشار ثم تمت مقارنة ثقة ثلاثة قراء في تشخيص التهابات العظم و الأنسجة الرخوة قبل و بعد اضافة صور خاصية الانتشار.<br/>نستنتج أن إضافة الصور المرجحة بالانتشار تضيف إلى الثقة التشخيصية للقراء مع اتفاق عال بين القراء ، والتي يمكن أن تكون ذات قيمة ملحوظة في المرضى الذين يمنع استخدام الصبغة في فحوصاتهم. يوفر قياس قيم معامل الانتشار الظاهري تقييما كميا لتقييم نخاع العظام ويضيف إلى خصوصية التصوير بالرنين المغناطيسي في تشخيص التهابات السكري. |
