| Summary, etc. | Background: Laparoscopic sleeve gastrectomy (LSG) has become one of the most commonly performed bariatric procedures and has been accepted by the American Society for Metabolic and Bariatric Surgery as a first-stage operation for morbidly obese patients. <br/>Aim and objectives: to assess the effect of gastric fixation on esophageal manometric values following laparoscopic sleeve gastrectomy.<br/>Patients and methods: The current randomized controlled clinical study conducted at Kasr Al Ainy hospitals, performed on 20 patients with laparoscopic sleeve gastrectomy (LSG) with gastropexy group and 20 patients LSG without gastropexy group. <br/> Result: there was statistically significant difference between LES resting pressure pre and DCI pre and post at LSG without gastropexy group. Food intolerance, vomiting and de novo GERD symptoms increased at LSG without gastropexy group and there was no significant difference in EWL % between both groups.<br/>Conclusion: Gastropexy step, although adds more to the operative time, but it preserved the esophageal manometric values such as LES resting pressure, LES intra-abdominal length and DCI. And reduced post-operative complications such as De novo GERD, vomiting and food intolerance. |

| Summary, etc. | أصبح استئصال المعدة بالمنظار (استئصال المعدة الطولي) أحد أكثر الإجراءات الجراحية التي يتم إجراؤها في جراحة السمنة، وقد تم قبوله من قبل الجمعية الأمريكية للجراحة الأيضية والسمنة كإجراء أولي للمرضى الذين يعانون من السمنة المفرطة.<br/> تعد السمنة أحد العوامل الرئيسية التي تساهم في الإصابة مرض الارتجاع المعدي المريئي، وهو حالة يحدث فيها ارتجاع لمحتويات المعدة إلى المريء مما يسبب أعراضاً و/أو مضاعفات، ويعد مرض الارتجاع المعدي المريئي شائعاً لدى المرضى الذين يسعون لإجراء جراحة السمنة.<br/>يعد استئصال المعدة الطولي من الإجراءات المفضلة عالميًا، ويتميز بميزة الحفاظ على استمرارية القناة الهضمية الطبيعية دون وجود وصلات معوية، كما أن له تأثيرات أقل على العمليات الأيضية. ومع ذلك، في الدراسات السريرية العشوائية، يرتبط استئصال المعدة الطولي بمعدل أعلى من أعراض مرض الارتجاع المعدي المريئي.<br/> لقد تم اعتماد استئصال المعدة بالمنظار كإجراء مستقل، مما يؤدي إلى فقدان الوزن الممتاز وتقليل الأمراض المصاحبة، وأصبح من أكثر الإجراءات الجراحية للسمنة شعبية. يؤدي تقليل حجم المعدة إلى تقليل حجم الطعام الذي يمكن تناوله، مما يؤدي إلى تقليل استهلاك الطعام. بالإضافة إلى ذلك، يؤدي الحجم المنخفض للمعدة الجديدة إلى تمدد أسرع، مما يحفز المستقبلات الميكانيكية على جدار المعدة.<br/> قد تكون هذه الآليات المحتملة هي التي دفعت إلى استخدام الجراحة التثبيتية للمعدة ، والتي تم اقتراحها كوسيلة لتثبيت وضع المعدة بعد استئصال المعدة بالمنظار وتحسين وظيفة المريء. من خلال ربط المعدة بالحجاب الحاجز، قد تقلل الجراحة التثبيتية من خطر حدوث مضاعفات مثل مرض الارتجاع المعدي المريئي وقد تؤثر أيضًا على القياسات المريئية في اختبارات قياس الضغط. |
