520 #3 - SUMMARY, ETC.
Summary, etc. Background: Plantar fasciitis is the predominant etiology of persistent heel pain in adults, impacting both physically active young persons and less active elderly individuals. It is caused by a long–term excessive strain on the plantar fascia. The cause of this condition can be attributed to either excessive use, commonly observed in runners and military personnel, or excessive strain, sedentary adults who are obese (with a body mass index greater than 30) and those who spend lengthy periods of time standing. Objective: To compare between the effect of shock wave therapy and low-level laser therapy on ankle joint painintensitylevel, pain pressure threshold, active ankle range of motion (dorsiflexion, plantarflexion, eversion and inversion), ankle stability and foot functional ability in patients with plantar fasciitis. Design: A randomized control design study. Subjects: Seventy-fivepatients from Ibny Beitak medical center–Egyptian Ministry of Health and Population and private clinic at October 6th city Methods: They were randomly assignedintothree equal groups. Group A(control group) Patients received conventional physical therapy program in the form of deep tissue massage, isometric exercises, stretching exercises, joint mobilization, ice application and some physical therapy home advice. This program was administered over twelve sessions three times per week. Group B (study group): Patients received low level laser therapy in addition to conventional physical therapy program for nine sessionsthreetimesperweek. Group C(study group): Patients received shock wave therapy in addition to conventional physical therapy program applied for a total of six sessions, one each week. Instrumentations: Visual analogue scale for pain intensity level, Pressurealgometer for pain pressure threshold of Quadratus plantae, Gastrocnemius and Soleus muscles, Digital goniometer for active range of motion of dorsiflexion, plantarflexion, eversion and inversion, Humac balance system for stability score between both sides, Foot and ankle ability measure questionnaire for activity and sport scores, Measurements were recorded before and after treatment. Results: There was no statistically significant difference observed between pain intensity level, pain pressure threshold of Quadratus plantae, Gastrocnemius, Soleus muscles, active ankle range of motion of dorsiflexors, plantar flexors, evertors and invertors, stability score, activity and sport scores before the study among the three groups. Additionally, post–study analysis revealed statistically significant difference between pain intensity level, pain pressure threshold of Quadratus plantae, Gastrocnemius, Soleus muscles, active ankle range of motion of evertors and invertors, activity and sport scores among the three groups but not significant neither for active ankle range of motion of dorsiflexors and plantar flexors nor stability score. Conclusion: Both shock wave therapy and low-level laser therapy was effective in treating patients with plantar fasciitis.
