Summary, etc. This study aimed to assess the visual , refractive , Aberrometric and keratometric outcomes as well as assessment of post-operative epithelial healing time and pain , after corneal collagen cross linking in female patients with keratoconus using two different techniques for epithelial removal , the conventional manual technique versus laser epithelial removal via Excimer laser transepithelial phototherapeutic keratectomy (t-PTK)<br/>Methods:<br/>Fourtyfemale patients ( 80 eyes ) were enrolled in this study , all of them had full ocular examination including slit lamb examination , assessment of un-aided and best corrected visual acuity , determining spherical equivalent and the manifest astigmatic error , as well as fundus examination , corneal imaging byPentacam [Allegro Oculyzer, WaveLight AG, Erlangen, Germany] was done to all patients to detect Maximum keratometric readings , corneal pachymetry , mean corneal astigmatism and assessment of corneal aberration map ( Coma , spherical , total corneal aberration) , then the patients were randomly divided into two equal groups , 20 patient (40 eyes) in each group. Group A The corneal epithelium was removed using t-PTKwithin a 7-mm zone to a depth of 50 micrometers , while group B The corneal epithelium was manually removed through mechanical debridement spanning an 8 mm diameter , then corneal collagen cross linking was done with the same technique (accelerated protocol for CXL) in both groups using riboflavin solution (0.1% ) that was applied to the central cornea every 2 minutes for a duration of 10 minutes then irradiationwith UVA light at 370 nm and an irradiance of 18 mW/cm² for 5 minutes for each eye will be done to all patients using (PESCHKE Trade CCL-VARIO Cross-linking; Swissmed, Gdańsk, Poland). pain was assessed in the first post operative hours and subsequent 4 days while Corneal epithelial healing time was assessed through slit lamp biomicroscopy till full healing, follow up was done at 1, 6 & 12 months postoperatively to assess and compare the visual, refractive, Aberrometric and keratometric outcomes in both groups.

Summary, etc. يعتبر مرض القرنية المخروطية من أكثر أمراض القرنيه شيوعاً, و يعتمد علاج القرنيه المخروطيه علي طبيعة المرحله المرضيه التي يعاني منها المريض،وتمثل تقنية تثبيت القرنية باستخدام الكولاجين البيني العلاج الرئيسي لتقوية أنسجة القرنية المتضررة،و يعتبر تشبع القرنيه بمادة الرايبوفلافين هو اهم عامل لإنجاح هذا الإجراء التداخلي ، مما آضفي أهميه لإزالة طبقةالخلايا السطحية للقرنية. ويمكن إزالة هذه الطبقة السطحيةعن طريقة الإزالة التقليديه اليدوية أوعن طريق الإيكزيمز ليزر، و يعتقد أنإزالة طبقة الخلايا السطحية للقرنية عن طريق الليزريوفر تحسنا في حدة الإبصار اكثر مما قد يتوفر عن طريق الإزاله اليدويه بسبب ما ينتج عن الليزر من مساواه لسطح القرنيه والذي من المتوقع ان يكون أكثر انتظاما باستخدام الليزر . و تهدف هذه الدراسة إلى مقارنة نتائج عمليات تثبيت القرنية بالكولاجين البينى بعد إزالة طبقةالخلايا السطحية للقرنية بالطريقة التقليديه اليدوية وعن طريق الإيكزيمز ليزر لعلاج حالات القرنية المخروطية المتقدمه في الإناث.
