Summary, etc. Volume expansion is the first line treatment in many cases of <br/>hemodynamic failure including mechanically ventilated patients. The main <br/>reason to administer volume is to increase preload and optimize cardiac <br/>output with subsequent effect being increasing oxygen delivery and <br/>reducing hypoxia. There are many indices that have been developed for <br/>prediction of fluid response in mechanically ventilated patients. <br/>Traditional, static methods to assess fluid responsiveness like central <br/>venous pressure and pulmonary artery occlusion pressure have lost their <br/>applicability in resuscitation protocols and can be unreliable in shock. <br/>Dynamic indices such as stroke volume variation and pulse pressure <br/>variation are useful for assessing fluid responsiveness in septic shock but <br/>are invasive; require patients to be in sinus rhythm. Simple bedside <br/>maneuvers like passive leg raise, capillary refill time, and end-tidal carbon <br/>dioxide provide opportunities to make an assessment of fluid <br/>responsiveness.

Summary, etc. للتشخيص المبكر والإنعاش المناسب لنقص التروية الدموية للأنسجة أهمية خاصة في إدارة الصدمة الإنتانية لتجنب تطور نقص الأكسجة النسيجية وفشل الأعضاء المتعددة. وقد اقتُرح تقييم تشبع الأكسجين الوريدي المختلط (SvO2) من قسطرة الشريان الرئوي كمؤشر غير مباشر على أكسجة الأنسجة الشاملة. يعكس SvO2 التوازن بين الطلب على الأكسجين والعرض. ويمثل انخفاض SvO2 استخلاصًا عاليًا للأكسجين (O2ER) للحفاظ على الأيض الهوائي وثبات VO2 استجابةً لانخفاض حاد في DO2. ومع ذلك، عندما ينخفض ​​DO2 إلى أقل من قيمة حرجة، يفقد O2ER القدرة على الحفاظ على VO2، ويظهر نقص أكسجة الأنسجة الشامل، كما هو موضح في حدوث الحماض اللبني.
