Background: A significant fraction of patients undergoing Taxanes suffer from peripheral neuropathy affecting hand strength, dexterity and function. Task-oriented traininghas been proven to improve the dexterity and strength of the hand.<br/>Purpose: to explore the effectiveness of task-oriented training (TOT) on grip strength, hand dexterity and upper extremity function in patients with taxanes-induced peripheral neuropathy. Design:Single-blind, envelop method randomized, controlled trial.Setting: Outpatient setting. Participants: Sixty female breast cancer patients were randomized equally either into group A(task-oriented training group) or group B (control group). Their ages were between 35 and 65 and all of them suffered from taxanes-induced peripheral neuropathy. Intervention: Over four weeks, with a frequency of three sessions per week, participants in group Aunderwent the task-oriented training program in addition to the traditional physical therapy (hand and wrist mobilization, strengthening and stretching exercises), while in group Breceived traditional physical therapy. Outcome measures: Hand grip strength was assessed using the Takei Analog Hand Grip Strength Dynamometer, hand dexterity was measured using the Nine-Hole Peg Test (NHPT), and upper extremity function was evaluated using the Arabic version of the Michigan Hand OutcomeQuestionnaire (MHOQ). All these outcome measures were assessed at baseline and after the four-weeks of interventions.<br/>Results: After the 4th week of intervention, the study found that Group A (task-oriented training) exhibited a non-significant decrease in hand grip strength (2.06%, p = 0.59), a substantial decrease in NHPT time (26.64%, p = 0.001), and a significant improvement in upper extremity function as reflected by MHOQ scores (77.63%, p = 0.001). In contrast, Group B (traditional therapy) showed a significant decrease in hand grip strength (21%, p = 0.001), no significant change in NHPT (6.39%, p = 0.12), and a significant improvement in MHOQ scores (48.08%, p = 0.001). Post-treatment comparisons between the groups revealed that Group A had significantly better outcomes, including improvement in hand grip strength (p = 0.01), a greater decrease in NHPT time (p = 0.01), and a larger increase in MHOQ scores (p = 0.001) compared to Group B. Overall, task-oriented training in Group A led to superior functional outcomes compared to traditional therapy.<br/>Conclusion: The task-oriented training (TOT) program, when combined with traditional hand exercises, proved effectiveness in improving fine motor skills,quality of life, hand dexterity and upper extremity function in breast cancer patients with taxane-induced peripheral neuropathy. It significantly slowed the decline in hand grip strength compared to traditional therapy alone, mitigating the effects of chemotherapy-induced peripheral neuropathy. |
