Summary, etc. Background; Kidney disease is a serious complication with negative impacts on health, quality of life and even life expectancy. Early detection and intervention are essential in the prevention and treatment of diabetic nephropathy (DN). Liver-type fatty acid-binding protein L-FABP is an intracellular fatty acid carrier protein which expresses mainly in the liver and kidney. L-FABP was associated with renal tubulointerstitial damage due to excessive reabsorption of free fatty acid. <br/>Aim and objectives; the main aim of this study was to detect the effect of Ramadan fasting on kidneys of type 2 diabetic patients through measuring urinary L-FABP level before, during and after the holy month of Ramadan. <br/>Subjects and methods; This cross-sectional analytic study was carried on 80 type 2 diabetic patients who insist to fast during the holy month of Ramadan. the study conducted in 2022 gregorian and 1443 hijri. While the fasting hours per day were about 15. The samples during Ramadan have been collected after 2 weeks of contineous fasting. The climate during Ramadan 1443 was warm and tolerable. <br/>Result; regardingL-FABPThe mean urinary L-FABP levels were 336.6, 299.7 and 318.8 unit when measured before, during and after fasting respectively.there was no statistically significant difference between the results of urinary L-FABP levels (P=0.243),Although there is a numerical decline in L-FABP levels during Ramadan which increased again after Ramadan but still below the levels before Ramadan.<br/>Regarding investigations for glycaemic control, the mean HbA1c before, during and after fasting were 8.9%, 8.4% and 8.3% respectively. Repeated-measurement ANOVA was run to check statistically significant change in HbA1c values, and we found it to be statistically significant. Comparing blood glucose levels by pairwise analysis using related samples Wilcoxon signed rank test confirmed significant change in levels before and during fasting. <br/>Urinary investigations analysis revealed a statistically significant difference between the results of the urinary A/C ratio obtained before, during and after fasting (p-value= 0.001) and the significance was also present comparing the results during and before fasting. <br/>Conclusion; In the current study, we revealed that there was no statistically significant difference between the results of urinary L-FABP levels when measured before, during and after Ramadan fasting respectively,Although there is a numerical decline in L-FABP levels during Ramadan which increased again after Ramadan but still below the L-FABP levels before Ramadan.Also L-FABP does not correlate to HbA1c, eGFR,creatinine,a/c ratio and BMI. Also, we found that fasting during the month of Ramadan does not affect kidney functions.Ramadan fasting has positive effects on T2DM patients as it reduces their blood glucose and HbA1C. . <br/>Further studies are needed with larger scales for confirming our results.

Summary, etc. تم التعرف على داء السكري(DM) منذ فترة طويلة باعتباره اضطراب التمثيل الغذائي الأكثر شيوعًا في جميع أنحاء العالم. داء السكري هو مجموعة من الأمراض التي تتميز في المقام الأول بارتفاع السكر في الدم المزمن بسبب عدم وجود عمل كاف للأنسولين. ينقسم مرض السكري تقريبًا إلى النوع الأول والنوع الثاني . <br/>رمضان هو الشهر التاسع من التقويم الإسلامي ويمارس المسلمون الصيام الإلزامي خلال هذا الشهر والذي قد يتراوح من 12 إلى 16 ساعة وأحيانًا أكثر في بعض البلدان حسب موقعها الجغرافي وموسمها . <br/>صيام شهر رمضان، أحد أركان الإسلام الخمسة، هو واجب إلزامي على جميع المسلمين البالغين الأصحاء. يجب على المسلمين خلال شهر رمضان الامتناع عن الأكل والشرب وتناول الأدوية عن طريق الفم، والتدخين من طلوع الفجر حتى غروب الشمس؛ لا توجد قيود على تناول الطعام أو السوائل بين غروب الشمس والفجر . <br/>يعد اعتلال الكلية السكري(DN) واحدًاً من أخطر مضاعفات الأوعية الدموية الدقيقة لمرض السكري من النوع 2، وهو أحد الأسباب الرئيسية للفشل الكلوي في المرحلة النهائية والوفاة لدى مرضى السكري. وهو أحد الأسباب الأكثر شيوعًا لمرض الكلى المزمن في جميع أنحاء العالم. <br/> البروتينالكبدي المرتبط بالأحماض الدهنية L-FABP هو بروتين حامل للأحماض الدهنية داخل الخلايا والذي يتم التعبير عنه بشكل رئيسي في الكبد والكلى. ارتبط L-FABP بالتلف الأنبوبي الخلالي الكلوي بسبب إعادة الامتصاص المفرط للأحماض الدهنية الحرة.<br/>تم الإبلاغ عن أن إفراز البول لـ(L-FABP) يوفر علامة حيوية محتملة للكشف المبكر عن اختلال وظائف الكلى سواء كان حاداً أو مزمنًا وبالتالي تحديد المرضى الذين من المحتمل أن يتعرضوا لتدهور وظائف الكلى في المستقبل. <br/> في دراسة مستقبلية حديثة، وجد أن L-FABP البولية أكثر حساسية من البيلة البروتينية في التنبؤ بتطور مرض الكلى المزمن .
