| Summary, etc. | The external, internal, and transversus abdominis muscle layers—as well as the facial sheath that surrounds them—compose the abdominal wall. The rectus abdominis muscle and the facial sheath that surrounds it are also a part of the central abdominal wall. Afferents passing via the transversus abdominis neuro-facial plane innervate this muscle wall.<br/>The somatic component of spinal nerve root T6–L1, which supplies the anterolateral abdominal wall, is anaesthetized by a transversus abdominis plane (TAP) block, a fascial plane block. In the neurofascial plane, which has less vascularity, we injected local anaesthetic between the transeversus and internal oblique abdominal muscles |

| Summary, etc. | تعد الولادات القيصرية من أكثر العمليات الجراحية شيوعا تُجرى يومياً ويصاحبها الام بعد الجراحة مما يضعف ارتباط الأم برضيعها ويزيد من فرص إصابة الأم بالجلطات الدموية نتيجةً لقلة الحركة.<br/>وتعد المواد الأفيونية ومضادات الالتهاب الستيرويدية من أركان نظام تسكين الألم المتعدد الأوضاع لضمان كفاءة التسكين وعلى صعيد اخر ترتبط المواد الأفيونية بعدد من الآثار الجانبية للأم بناءاً على جرعتها مثل تثبيط الجهاز التنفسي والغثيان والقيء والنعاس وفرط التألم إلى جانب زيادة خطورة انتقالها عبر المشيمة.<br/>وبالرغم من أن الإحصار النخاعي يعد الطريقة الأمثل لتسكين ألم ما بعد الولادة القيصرية إلا ان الإحصار الطرفي مثل تخدير سطح العضلة المستعرضة البطنية وتخدير العضلة المربعة القطنية أصبح يلقى اهتماماً كبيرا لما يسببه الإحصار النخاعي من اعراض جانبية كثيرة مثل انخفاض ضغط الدم والصداع والاحتباس البولي والحكة.<br/>ويتم تخدير سطح العضلة المستعرضة البطنية عن طريق حقن مخدر موضعي بين العضلة المستعرضة البطنية والعضلة البطنية الداخلية المائلة.<br/>هذه الدراسة تؤكد أنه من الآمن والمقبول تخفيف 150 مجم من عقار البيوبيفيكين 0,5 % (30 مللي) مع 90 مللي محلول ملحي لتكون محصلة الحقن 60 مللي بكل جانب من البطن في تخدير السطح المستعرض البطني للحوامل اللاتي يخضعون للولادات القيصرية لتسكين الألم بعد العملية لما لهذه الطريقة من مفعول ممتدة تأثيرها لفترة زمنية طويلة.<br/> |
