| Summary, etc. | Aim: The aim of this study was to compare the effect of Injectable Platelet-Rich Fibrin (i-PRF) mixed with nNano-bioactive glass (45S5 nBG) powder versus i-PRF alone, on postoperative pain, and preservationof pulp vitality in permanent molars with closed apices diagnosed with symptomatic irreversible pulpitis (SIP).<br/><br/>Methods: Thirty-two patients diagnosed with SIP in permanent mandibular molars with preoperative pain, were randomly allocated into two groups according to the dressing material used after full pulpotomy; group I: i-PRF alone and group II: I-PRF mixed with 45S5 nBG after full pulpotomy. The intensity of postoperative pain intensity (PI) was measured using modified visual analogue scale (modified-VAS) after 24 hours, and daily up to the 7th day. Moreover, the clinical and radiographic success outcomes after 1,3 and 6 months were recorded. Clinical outcomes were evaluated using thermal (cold) test, EPT and percussion test; whereas radiographic outcomes were assessed and recorded according to a modification of Strindberg’s criteria. All demographic, baseline, and outcome data were collected and statistically analyzed. <br/><br/>Results: PI outcomes were measured in terms of mean, standard deviation (SD) and 95% Confidence Interval (CI) of pain score. Intervention group showed a significantly lower pain score compared to the control group till the 4th day (p<0.05). Nevertheless, at 5th, 6th and 7th day, p= 0.239, p=1.00, p=1.00 respectively, there was insignificant difference between the tested group pain score. Clinical success outcomes showed; insignificant difference between the control and intervention group at 1 month and 3 months (p=1.00). After At6 months, the control group showed 4 cases (25%) with (cold) test and EPT failure compared to the intervention group with significant difference at p=0.038, in addition to frequency and percentage of pain on percussion at 6 months, showed a significant difference between the control and intervention group at p=0.018. Moreover, radiographic outcomes showed insignificant difference between the control and intervention group at 1 month and 3 months p=1.00. However, after at6 months, the control group showed 4 cases (25%) with radiographic failure compared to the intervention group with significant difference at p=0.038.<br/><br/>Conclusion: Within the limitations of the present study, it can be concluded that i-PRF alone or mixed with 45S5 nBG could serve as a biological scaffold after pulpotomy in permanent mandibular molars with closed apices diagnosed with SIP. Post-operative pain in both groups showed a reduction in intensity within the 1st week after the procedure, where, PI outcomes were significantly lower in the intervention group at 4th day. Sensibility, pPercussion testing and radiographic evaluation showed significant difference in favor of the i-PRF mixed with 45S5 nBG group at 6 months. |
