| Summary, etc. | Introduction: Bladder cancer is a global health problem worldwide and the ninth most common cancer.The most appropriate treatment algorithm form muscle-invading disease remains controversial. Although radical cystectomy and urinary diversion has been the mainstay for treatment for decades, organ preserving regimens using predominantly multimodality therapy, consisting of trans urethral resection (TURBT) followed by irradiation with concurrent chemotherapy, are emerging as viable proven alternatives in a subset of patients <br/>Patient and methods:One hundred fourteen MIBC patients randmoized between two arms conventional and hypofractionated radiotherapy.Results: The median followup was 33 monthes,3 years OS of two arms were are 78%,and 77.7%respectively.Three years DFS are 74.4% and 72.4% respectively.<br/>Conclusion: A hypofractionated schedule is good alterantive to conventional schedule with regard to OS, DFS , locoregional control and toxicity, .Longer follow up is needed to adopt hypofractioantion as a standard of care for bladder preservation in patients with locally advanced bladder cancer. |

| Summary, etc. | هذه دراسة مستقبلية بحثيه بين جرعتين مختلفتين من العلاج الاشعاعي الأورام المثانة بقسم العلاج الإشعاعي بالمعهد القومي للأورام في الفترة من 2019 الى 2022 على 114 مريض.<br/>يعد الاستئصال الجذري الأورام المثانة هو العلاج الرئيسي لهذه الحالات ولكن العلاج الإشعاعات متزامن مع العلاج الكيماوي أفضل اختيار في حالة رفض المريض اجراء العملية او وجود اسباب تمنع اجراءها. تستهدف هذه الدراسة دراسة جرعتين مختلفتين من العلاج الاشعاعي لهذه الحالات حيث يتلقى المريض العلاج الاشعاعي في فترة اربعة اسابيع أو ستة اسابيع. تهدف الدراسة إلى تقليل فترة العلاج مع عدم تأثير ذلك على المريض وعلى نتائج العلاج.<br/>تم تقسيم المرضى عشوائياً إلى مجموعتين:<br/>المجموعة أ: 66على مدار 33 جلسة لمدة سبعة اسابيعGy: العلاج الإشعاعي بجرعة علاجية.<br/>المجموعة ب: 55على مدار 20 جلسة لمدة اربعة اسابيعGy: العلاج الإشعاعي بجرعة علاجية.<br/>تلقى جميع المرضى في المجوعة أ العالج باستخدام تقنيات العالج الشعاعي متغير الشدة <br/>تم توزيع المرضى بشكل عشوائي بين جرعتين من العلاج الإشعاعي التقليدي والعلاج الإشعاعي الناقص التجزئة كان متوسط المتابعة 33 شهرًا، وكانت معدل الاعمار لمدة 3 سنوات للجرعتين 78%، و77.7% على التوالي. وكانت نسبة معدل لأعمار بدون مرض لمدة ثلاث سنوات 74.4% و72.4% على التوالي بين الجرعتين.<br/>فيما يتعلق بالأثار الجانبية الحادة والمزمنة للعلاج الشعاعي، تم تسجيل أثار جانبية على مقياس للجهاز الهضمي الجهاز التناسلي من الدرجة الثانية على مقياس RTOG ولم يكن هناك اختلاف بين الجرعتين<br/>وقد ادى تلقى الرضى العلا ج الكيماوي قبل الجلسات الى تقليل نسبة حدوث ثانويات من الأورام بنسبة تتعدى 90% |
