Summary, etc. background<br/>The aim of this study is to explore the value of diffusion tensor imaging (DTI) parameters to differentiate between benign and malignant breast lesions compared to diffusion weighted MRI and Dynamic Contrast Enhanced imaging. <br/>Patients and methods<br/>This study was conducted in the Radiology Department of the national cancer institution during the period from 2018 till 2024.This prospective observational cohort research was performed on 64 females aged 26–75 years old featured with breast complaints and/or abnormal sono-mammographic findings. <br/>Conventional and dynamic MRI sequences were done for all patients on a 1.5 Tesla MRI (Philips Achieva XR) in the standard prone position, using a 4-channel dedicated breast coil. <br/>Quantitative analyses were done using a dedicated workstation (Philips Medical Systems National Cancer Institute Cairo University). ADC, MD and FA, the localization of ROI’s were determined in consensus. The quantitative analysis of DWI and DTI, ADC and FA maps were created automatically by the imaging console. <br/>Results:<br/>•This study included 64 female patients with 68 breast lesions (bilateral lesions in some patients) . Frome table 9: 14 patients had a positive family history. The age of our patients varied from 26 to 75 years, with the mean age was 47.4±10.7. 63 patients (98.4%) were represented by mass lesions. <br/>•Most of the mass with heterogeneous enhancement patterns were in malignant lesions (37 lesions, 77.08%) that were statistically significant than benign lesions (11 lesions, 16.18%) . <br/>•dynamic MRI diagnosed 57 malignant lesions and 11 benign lesions while the histopathological results of our 68 lesions were as follows: 58 pathologic proven lesions to be malignant and 10 were pathologically proven benign lesions, correlation with histopathological results demonstrated that dynamic contrast enhancement MRI produced sensitivity, specificity and accuracy of 94.83 %, 80.0% and 92.65%, respectively.<br/>•MRI diffusion ADC values, benign ADC conclusion was present in 15 lesions and malignant conclusion was present in 53 lesions, while the histopathological results of our 68 lesions were as follows: 58 pathologic lesions proven to be malignant and 10 were pathologically proven benign lesions.<br/>•ADC values were significantly lower in malignant (1.054±0.284× 10−3 mm2/s) than in benign lesions (1.388±0.228× 10−3 mm2/s), with a cut-of value of 1.21× 10−3 mm2/s; this gives a sensitivity of 87.93%, specificity 80.0% and accuracy 86.76%.<br/>•DTI FA value, benign FA conclusion was present in 10 lesions and malignant conclusion was present 58 lesions, while the histopathological results of our 68 lesions were as follows: 58 pathologic lesions proven to be malignant and 10 were pathologically proven benign lesions.<br/>•FA values were significantly higher in malignant tumors (0.29±0.453× 10−3 mm2 /s) than benign lesions (0.129±0.032× 10−3 mm2/s) with a cut-of value of 0.15 × 10−3 mm2, gives sensitivity, specificity, and accuracy of 98.28%,90.0%, and 97.06%, respectively.<br/>•DTI MD value, benign MD conclusion was present in 11 lesions and malignant conclusion was present in 57 lesions, while the histopathological results of our 68 lesions were as follows: 58 pathologic lesions proven to be malignant and 10 were pathologically proven benign lesions.<br/>•MD values were significantly lower malignant tumors (0.29±0.453× 10−3 mm2 /s) than benign lesions (1.75±0.670× 10−3 mm2/s) with a cut-of value of 1.5 × 10−3 mm2 /s,. Mean MD gives sensitivity, specificity, and accuracy of 96.55%,90.0%, and 95.59%, respectively.<br/>•Collectively, analyzing data of the present study, among DTI parameters FA showed the highest diagnostic accuracy with sensitivity (98.28%) and specificity (90.00%). Following, MD with specificity and sensitivity of (96.55%) and (90.00%), respectively, DCE sensitivity and specificity were (94.83%) and (80.00%), respectively, the MRI diffusion ADC value in our study show sensitivity and specificity (87.93%) and (80.00%), respectively.

Summary, etc. الخلفية العلمية و الهدف من الرساله:<br/>الهدف من هذه الدراسة هو استكشاف قيمة التصوير بمعامل الانتشار الموزون للتمييز بين أورام الثدي الحميدة والخبيثة مقارنة بالتصوير بالرنين المغناطيسي الإنتشاري والتصوير بالرنين بالصبغة الديناميكى.<br/>طرق البحث:<br/>شملت الدراسة 64مريضة، عانوا من إجمالي 68ورم في الثدي، بما في ذلك مرض متعدد المراكز متعدد البؤر. تم تصميم هذه الدراسة لتقييم ما إذا كان التصوير بمترر الانتشار يمكن أن يوفر دقة تشخيصية فائقة في التمييز بين الآفات الحميدة والخبيثة و تم عمل فحص رنين مغناطيسي ديناميكي ورنين باستخدام معامل الانتشار ومعامل الانتشار الموزن ومقارنة النتائج ببعضها البعض وبنتائج الباثولوجي<br/>النتائج:<br/>•فيما يتعلق بفحص الرنين بالصبغة ٣٧ عينة ظهر بها كتل غير متجانسة فى توزيع الصبغة ومع فحص الغدد اليمفاويه و العلامات المصاحبة اثناء الفحص تم تشخيص٥٧ عينة عالية الاحتمال ان تكون خبيثه و ١١ عينة عاليه الاحتمال ان تكون حميده<br/>•كانت نتيجة التصوير بمعامل الانتشار الحميدة موجودة في ١٥ عينة وكانت نتيجة التصوير بمعامل الانتشار الخبيثة موجودة في ٥٣ عينة. <br/>•كانت نتيجة التصوير بالرنين المغناطيسى الموزن بمعامل تباين الانتشار الحميدة موجودة في ١٠ عينات من المرضى وكانت نتيجة انتشار التصوير بالرنين المغناطيسي الخبيثة موجودة في ٥٨ عينة<br/>•كانت نتيجة التصوير بالرنين المغناطيسى الموزن بمعامل الانتشار وقياس متوسط الانتشارية الحميدة موجودة في ١١ عينات من المرضى وكانت نتيجة انتشار التصوير بالرنين المغناطيسي الخبيثة موجودة في ٥٧ عينة.<br/>•وبمطابقة الأرقام والنتايج بنتيجة عينات الباثولوجى للحالات وجد ان التصوير بالرنين المغناطيسى الديناميكى أعطى نسبه حساسية ٩٤.٨٣% ونسبة خصوصية ٨٠% ونسبة دقة ٩٢.٦٥% بينما التصوير بمعامل الانتشار اعطى نسب ٨٧.٩٣% و ٨٠% و ٨٦.٧٦% على التوالى وكانت نتيجه استخدام معامل الانتشار الموزون بمعامل التباين هى ٩٨.٢٨% و ٩٠% و ٩٧.٠٦% على التوالى وباستخدام متوسط الانتشارية كانت النتائج كالتالى ٩٦.٥٥% و ٩٠% و ٩٥.٥٩% نسبة الحساسية والخصوصية والدقة على التوالى<br/>•حقق التصوير بالرنين المغناطيسى باستخدام معامل الإنتشار الموزن نسب ونتائج أعلى من التصوير باستخدام خاصية الانتشار فقط وأعلى من فحص الرنين المغناطيسى بالصبغة منفرد. واستخدامها معا مجتمعين زاد من نسبه دقة وحساسية التشخيص.
