Summary, etc.	Background: Three million patients with Multiple sclerosis (MS) globally <br/>participate in a reduced physical activity, leading to de-conditioning and a <br/>decline in functional ability. Aerobic arm training is easily accessible, has <br/>advantages especially in advanced stage of MS and provides an alternative option <br/>in various circumstances, but there is no clear evidence summarizing the <br/>available limited literature discussing it. Objective: To systematically review <br/>and critically appraise the trials that addressed the effect of arm cycling on <br/>fatigue and functional outcomes in patients with MS. Methods: Six electronic <br/>databases were investigated for relevant clinical trials for arm cycling in <br/>multiple sclerosis disease with no language restriction, no time limitation, only <br/>human studies and only clinical trials studies were included. Methodological <br/>appraisal was conducted using PEDro scale and the outcomes were analyzed <br/>qualitatively. Results: five included studies involved 166 patients with multiple <br/>sclerosis; Four trials considered as good quality studies, and one study was <br/>considered as a fair quality study. The evidence provided that adding arm <br/>cycling could achieve a significant improvement in fatigue, cardiorespiratory <br/>fitness, heart rate, power, aerobic capacity, manual dexterity and quality-of-life <br/>in some measures. Conclusion: Moderate to strong evidence reinforcing that <br/>adding arm cycling could achieve an improvement in fatigue and functional <br/>outcomes in patients with MS. The available literature shows that there is a need <br/>for longer treatment time and higher quality randomized controlled trials with <br/>large participants to support the available evidence about arm cycling in patients <br/>with MS.

Summary, etc.	الخلفية: ثلاث ملايين من مرضى التصلب المتعدد حول العالم يشاركون فى نشاط بدني منخفض، مما يؤدي إلى ضعف اللياقة البدنية وانخفاض القدرة الوظيفية. يعد تمارين التبديل باستخدام الذراعين سهلة المنال، ولها فوائد خاصة في المرحله المتقدمة من لمرض التصلب المتعدد ، و يقدم خيارًا بديلًا في ظروف مختلفة، ولكن لا يوجد دليل واضح يُلخص الدراسات المحدودة المتاحة التي تتناولها. الهدف: لتلخيص بطريقه ممنهجة وتقييم بشكل نقدى الدراسات التى تناولت تأثير تمارين التبديل باستخدام الذراعين على التعب والنتائج الوظيفية لدى المرضى التصلب المتعدد. الطريقة: تم البحث في ست قواعد بيانات إلكترونية للحصول على التجارب السريرية ذات الصلة التبديل باستخدام الذراعين في مرض التصلب المتعدد، بدون قيود لغوية أو زمنية، مع اختيار الدراسات الخاصة بالبشر و المصنف كدراسات تجارب سريرية فقط. و قد أُجري التقييم المنهجي باستخدام مقياس بيدرو وحُللت النتائج نوعيًا. النتائج: شملت الخمس دراسات المُدرجة١٦٦ مريضًا مصابا بالتصلب المتعدد؛ كانت أربع تجارب ذات جودة جيدة وتجربة واحدة ذات جودة متوسطة.توضح الأدلة أن إضافة تمارين التبديل باستخدام الذراعين يُمكن أن تُحقق تحسنًا ملحوظًا في التعب، واللياقة القلبية التنفسية، ومعدل ضربات القلب، والقوة، والقدرة الهوائية، والبراعة اليدوية، وجودة الحياة باستخدام بعض القياسات الاستنتاج: وجدت أدلة متوسطة إلى قوية تُعزز فكرة أن إضافة تمارين التبديل باستخدام الذراعين يُمكن أن تُحسّن من التعب والنتائج الوظيفية لمرضى التصلب المتعدد. تُظهر الدراسات المتاحة أن هناك حاجة إلى فترة علاج أطول فى تجارب سريرية عشوائية مُحكمة عالية الجودة، مع عدد كبير من المشاركين، لدعم الأدلة المتاحة حول تمارين التبديل باستخدام الذراعين لدى مرضى التصلب المتعدد.
