| Summary, etc. | Total 36 cases are divided into two groups: group (I) operated by intra<br/>medullary nail and group (II) operated by minimally invasive osteosynthesis with a <br/>compression plate. Pre-operative variables included the patient`s age, the side of <br/>the fracture, mode of trauma and pattern of fracture. Postoperative variables were <br/>wound problems, knee pain and the time to union of the fracture and the <br/>Radiographic Union Score for Tibial Fractures for the Assessment of Tibial <br/>Fracture Healing (RUST) and time of weight bearing. <br/>We found no significant difference in the pre-operative variables or in the time to <br/>union in the two groups. However, there was significant differences in operative <br/>data as IMN group showed more operative time and radiation shots and less blood <br/>loss compared to MIPO group. Also, IMN group showed early weight bearing <br/>more than MIPO group. IMN cases weight bearing begun from day 12 up to day <br/>21. |

| Summary, etc. | تحدث كسور خارج المفصل القريب في الساق فقط في ٥% الى ١١% من جميع كسور الساق. نظرا لأنها غالبا ما تكون نتيجة نقل الطاقة المرتفع (على سبيل المثال، تصادم سيارات المشاه)، فقد يؤدي وضع غير مستقر للغاية مع تفتيت العظام وتلف الأنسجة الرخوة واسعة النطاق.<br/>كسور الساق خارج مفصل الساق البعيد هي إصابات مدمرة تحدث عادة بسبب آليات الطاقة العالية مثل السقوط من المرتفعات أو حوادث السيارات. قد تحدث أيضاً من آليات منخفضة الطاقة، والتي تظهر في الإصابات الدورانية حول الكاحل.<br/>مقارنه نتائج سوء التئام وتحمل الوزن المبكر للمسمار النخاعي (مع أو بدون استخدام شريحه ضغط ديناميكية صغيرة ٣.٥ مم) مع شريحة عن طريق التدخل الجراحي البسيط عن طريق الجلد في المرضى الذين يعانون من كسور القصبه القريبه من مفصل الركبه والقريبة من مفصل الكاحل في البالغين في دراسة مستقبلية.<br/>تشمل دراستنا ما مجموعه ٣٦ حالة تنقسم إلى مجموعتين: المجموعة الاولي و هي المسمار النخاعي والمجموعة الثانية وهي شريحة عن طريق التدخل الجراحي البسيط عن طريق الجلد. متغيرات ما قبل الجراحة شملت عمر المريض والجانب ونمط الكسر ووضع الصدمة. كانت متغيرات ما بعد الجراحة هي مشاكل الجرح وآلام الركبة ووقت التئام الكسر وفقاً لنقاط الالتئام الإشعاعي لكسور القصبه لتقييم شفاء كسر القصبه، وسوع الالتئام وزمن التحميل.<br/>لم نجد فرق كبير في معدل التئام الكسر بين مجموعتين. ومع ذلك، أظهرت مجموعه للمسمار النخاعي تحمل الوزن مبكرا أكثر من مجموعه شريحة عن طريق التدخل الجراحي البسيط ، حالات المسمار النخاعي بدأت تحمل الوزن منذ اليوم ١٥ وحتى اليوم ٢١.<br/>لم نعثر على أي تشوهات زاوية في المتابعة بفيلم اشعه سينيه طويل علي الأطراف السفلية، وتم تحقيق التوافق الجيد. استخدام شريحه للشظيه يعطي محاذاة جيدة للساق ويقلل من وقت العمليه المستغرق في استعدال الكسر واستخدام الأشعة السينية.<br/>معظم المضاعفات، مجموعة المسمار النخاعي كانت الام في الركبة في موقع دخول المسمار واستمرت لمدة أقل من شهر. شكت حالة واحدة فقط من مجموعة شريحة عن طريق التدخل الجراحي البسيط من آلام في الكاحل وكان معظمها لا يتبع تعليمات العلاج الطبيعي للكاحل. |
