| Summary, etc. | Introduction<br/>The transoral endoscopic thyroidectomy vestibular approach (TOETVA) is the latest proposed operation and is currently the only “scarless” or more appropriately “hidden and invisible scar” technique. TOETVA follows all the steps of a conventional thyroidectomy having the advantage of surgical access close to the thyroid area. Additionally, the central trocar for the camera allows a good view of both the thyroid loggias. The aim of this work was to assess the feasibility of TOETVA in national cancer institute Egypt to assess operative time, complication safety, reliability and limitation of the new technique. <br/>aim of the work <br/>The aim of this study was to assess the feasibility, safety, operative outcomes, and complications of TOETVA in a tertiary care setting in Egypt.<br/>Patients and Methods<br/>A prospective case series of 31 patients undergoing TOETVA at the National Cancer Institute, Cairo University, was conducted between 2023 and 2024. All patients had Bethesda II–V nodules ≤6 cm. Primary outcomes included operative time, complication rate, and conversion to open surgery. Secondary outcomes included RLN injury, hypoparathyroidism, and oncological resection completeness.<br/>Results<br/>TOETVA was successfully completed endoscopically in 30 of 31 patients (96.8%). The median operative time was 150 minutes. The complication rate was 19.4%, with transient recurrent laryngeal nerve (RLN) injury in 1 case (3.2%) and no permanent complications. One conversion to open surgery occurred due to intraoperative air embolism. Five patients (16.1%) had papillary thyroid carcinoma, all resected with clear margins. Higher BMI was significantly associated with increased complication risk (p = 0.035).<br/>Conclusion<br/>TOETVA can be safely and effectively adopted in centres without prior laparoscopic or robotic experience. With appropriate training and patient selection, this technique is a viable alternative to conventional thyroidectomy, offering superior cosmetic outcomes and acceptable complication rates. |

| Summary, etc. | يُعد استئصال الغدة الدرقية عبر الفم باستخدام المنظار (TOETVA) من أحدث التقنيات الجراحية المبتكرة، ويتميز بكونه الأسلوب الوحيد الذي لا يترك ندبة ظاهرة على الجلد، بل يحقق نتائج تجميلية متميزة دون التأثير على جودة الجراحة. تهدف هذه الدراسة إلى تقييم مدى إمكانية تطبيق هذه التقنية في مصر من حيث الوقت الجراحي، معدل المضاعفات، معدل التحويل للجراحة المفتوحة، وسلامة وحدود هذا الأسلوب.<br/>تم إجراء دراسة مستقبلية شملت ٣١ مريضًا بالمعهد القومي للأورام، جامعة القاهرة، بين عامي ٢٠٢٣ و٢٠٢٤، وكانت كل الحالات تحتوي على عقيدات درقية مصنفة من الفئة الثانية حتى الخامسة وفق نظام Bethesda، ولا تتجاوز ٦ سم. تضمنت النتائج الأساسية: الوقت الجراحي، معدل المضاعفات، ومعدل التحويل للجراحة المفتوحة. كما تم تقييم سلامة الأعصاب الحنجريّة المتكررة، اضطرابات الغدة الجار درقية، وكفاءة الاستئصال الأورامى.<br/>تمت الجراحة بالمنظار بالكامل في ٣٠ حالة (٩٦.٨٪)، بمتوسط وقت جراحي ١٥٠ دقيقة. بلغت نسبة المضاعفات ١٩.٤٪، وتضمنت حالة إصابة مؤقتة بالعصب الحنجري (٣.٢٪)، دون تسجيل أية مضاعفات دائمة. تم تحويل حالة واحدة فقط إلى الجراحة المفتوحة بسبب حدوث انصمام هوائي أثناء العملية. وتم اكتشاف سرطان درقي حليمي في خمس حالات (١٦.١٪)، وجميعها تم استئصالها بهوامش آمنة. كما تبين أن ارتفاع مؤشر كتلة الجسم يرتبط بزيادة خطر المضاعفات (قيمة p = 0.035). |
