Summary, etc. Background: Pulmonary function tests (PFTs) are noninvasive tests which examine the functionality of the lungs and permit accurate assessment of the functional state of the respiratory system. Core stability is the capability to strengthen the lumbopelvic complex and transfer forces from the upper to lower limbs of the body while maintaining the spine in a neutral position. This review aimed to provide conclusive evidence about the effect of core stabilization exercises on pulmonary functions. Methods: Seven databases (Scopus, EMBASE, Web of Science, PubMed, EBSCO host, SAGE, and Cochrane library) were searched from 2000 to 2024. Randomized controlled trials investigated pulmonary functions before and after application of core stability exercises. The two reviewers, Mohamed Ahmed Omar Mohamed and Ali Mohamed Ali Ismail independently screened articles and assessed the risk of bias. Data extraction was performed by the first reviewer and crosschecked by a second reviewer. The quality of findings was assessed and rated according to Version 2 of the Cochrane risk-of-bias tool for randomized trials (RoB 2) guidelines. Forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FEV1/FVC ratio, peak expiratory flow rate (PEFR), and maximal oxygen uptake (VO2max) outcomes were extracted and synthesized qualitatively and quantitatively. Results: out of the seven studies, two studies investigated the effect of core stability exercises on VO2max. Meta-analysis of the results demonstrated a non-significant effect of core stability exercise on VO2max but with considerable heterogeneity. Two studies investigated the effect of core stability exercises on PEFR; Meta-analysis of the results demonstrated a significant effect on PEFR with no heterogeneity. Six studies investigated the effect of core stability exercises on FVC; the analysis of the results demonstrated significant effect of core stability exercise on FVC but with considerable heterogeneity. Four studies investigated the effect of core stability exercises on FEV1; The analysis of the results demonstrated significant effect of core stability exercise on FEV1, with no heterogeneity. Three studies investigated the effect of core stability exercises on FEV1/FVC. The analysis of the results demonstrated significant effect of core stability exercise on FEV1/FVC. Conclusion: In the current study, we found that core stability exercises have a significant effect on FVC, FEV1, FEV1/FVC, PEFR unfortunately, we did not find a significant effect on VO2max because of limited studies included in the meta-analysis investigating the effect of core stability on VO2max (two studies only). The current level of evidence supports the effectiveness of core stabilization exercises and pelvic floor exercises on pulmonary functions. but further clinical trials are required to clinically support the effect of core stabilization exercises and pelvic floor exercises on pulmonary functions.
