| Summary, etc. | Background: Diabetic neuropathy is a debilitating complication of diabetes, <br/>significantly impacting electrophysiological function and quality of life (QoL) <br/>Purpose: This study was conducted to investigate the effectiveness of high-<br/>power laser therapy (HPLT) on electrophysiological parameters and the <br/>quality of life (QoL) in patients with diabetic neuropathy. Methods: Forty-four <br/>patients of both sexes with diabetic neuropathy secondary to type 2 Diabetes <br/>Mellitus aged from 50 to 60 years old participated in this study. Patients were <br/>randomly divided into two equal groups; group (I) received HPLT in addition <br/>to conventional physical therapy and group (II) received the same conventional <br/>physical therapy. Treatment was conducted two session per week for six weeks. <br/>Neuropathic pain was assessed using visual analogue scale (VAS), neuropathy <br/>severity was assessed by modified neuropathy disability score (mNDS), <br/>electrophysiological parameters (latency, amplitude and conduction velocity of <br/>both sural and peroneal nerves of the right side)) were assessed using <br/>electromyography device. Quality of life was assessed using neuropathy <br/>specific quality of life (Neuro-QoL) questionnaire. All outcome measures were <br/>assessed pre and post treatment. Results: There were no statistically significant <br/>differences between both groups in pretreatment mean values of all measured <br/>variables. There was a statistically significant decrease in mean value scores of <br/>VAS, mNDS, Neuro-QoL and latency of both peroneal and sural nerves, while <br/>there was a statistically significant increase in mean value scores of amplitude <br/>and conduction velocity of both sural and peroneal nerves of the right side in <br/>both groups after treatment. There were significant differences in all measured <br/>variables between both groups after treatment, favoring the study group. <br/>Conclusion: It was concluded that high power laser therapy has more positive <br/>effect than conventional physical therapy alone in reducing pain and <br/>neuropathy severity and improving peripheral nerve function and QoL in <br/>patients with diabetic neuropathy. |

| Summary, etc. | الخلفية: يُعد الاعتلال العصبي السكري من مضاعفات مرض السكري المُنهكة، ويؤثر بشكل كبير على الوظيفة الكهربية الفيزيولوجية وجودة الحياة. الغرض : الغرض من هذه الدراسةُ هو معرفة تأثير العلاج بالليزر عالي القوة على المعايير الكهروفسيولوجية وجودة الحياة فى مرضى الاعتلال العصبى السكرى. الطريقة :أجرى البحث على اثنين وأربعين مريضا من مرضى الاعتلال العصبى السكرى من كلا الجنسين، وقد تراوحت أعمارهم بين خمسين وستين عاما. وقد تم تقسيم المرضى عشوائيا إلى مجموعتين متساويتين (مجموعات الدراسة والضابطة). تلقت مجموعة الدراسة ليزر عالى القوة بالإضافة إلى برنامج العلاج الطبيعي التقليدى، و تلقت المجموعة الضابطة نفس برنامج العلاج الطبيعي التقليدى فقط. وقد أجرى برنامج العلاج الطبيعي مرتين في الاسبوع لمدة ستة أسابيع.التقييم: خضع المرضى في المجموعتين لتقييم شدة الألم باستخدام مقياس التماثل البصري ، وتم استخدام مقياس درجة إعاقة الاعتلال العصبى المعد للتقييم شدة الاعتلال العصبى السكرى، وتم استخدام استبيان جودة الحياة الخاص بالاعتلال العصبى لتقييم جودة الحياة واستخدام جهاز رسم العضلات لتقييم المعايير الكهروفسيولوجية ( الكمون وسعة وسرعة توصيل العصب). و تم إجراء التقييم لجميع المتغيرات قبل وبعد البرنامج العلاجي. النتائج: أوضحت المعالجة الاحصائىة للنتائج عدم وجود فروق ذى دلالة إحصائية فى متوسط قيم درجات جميع المتغيرات المقاسة بين المجموعتين قبل العلاج . بينما أسفرت النتائج عن وجود انخفاض ذو دلالة إحصائية فى متوسط قيم درجات مقياس شدة الألم, مقياس درجة إعاقة الاعتلال العصبى المعدل، درجات استبيان جودة الحياة الخاص بالاعتلال العصبى والكمون للعصب الربلى والشظوى للطرف السفلي الأيمن وزيادة ذات دلالة إحصائية في متوسط قيم درجات سعة وسرعة التوصيل للعصب الربلى والشظوى للجانب الأيمن في كلا من المجموعتين بعد العلاج مع وجود فارق ذو دلالة إحصائية في جميع المتغيرات المقاسة بين المجموعتين بعد العلاج لصالح مجموعة الدراسة. الخلاصة: إضافة العلاج بالليزر عالي القوة للعلاج الطبيعي التقليدى له تأثير اكثر إيجابي من العلاج الطبيعى التقليدى بمفرده فى تقليل الألم وشدة الاعتلال العصبي وتحسين وظيفة العصب وجودة الحياة فى مرضى الاعتلال العصبي السكري . |
