| Summary, etc. | Background and Purpose: Chronic supraspinatus tendinitis is a common cause of shoulder pain. Although it is a common condition, the pathophysiology is not fully understood. The purpose of this study is to Investigate and compare the effect of Radial Shockwave and the effect of Ultrasound Diclofenac Na+ Phonophoresis on the thickness of Supraspinatus tendon, pain intensity of shoulder joint, Rang of motion of the shoulder joint and upper limb function in management of Chronic Supraspinatus Tendinitis. Materials and Methods: Sixty-nine patients with chronic supraspinatus tendinitis participated in this study. Subjects were subdivided into three groups, Twenty-three in each group. Group A received radial shockwave and conventional physiotherapy, group B received ultrasound phonophoresis and the conventional physical therapy, and group C received the conventional physical therapy. The outcome measures were the thickness of Supraspinatus tendon, pain intensity of shoulder joint by (VAS), upper limb function by (quick dash) and range of motion of the shoulder joint (abduction, flexion, and external rotation) Results: All three groups improved each measured variable when compared pre and post-intervention (p=0.001). However, the mean difference between group A and B was -1. There was a significant decrease in VAS of group A compared with that of group B (p = 0.005). The mean difference between group A and C was -1.82. There was a significant decrease in VAS of group A compared with that of group C (p = 0.001). The mean difference between group B and C was -0.82. There was a significant decrease in VAS of group B compared with that of group C (p = 0.02). Conclusion: All Groups was significally improved but Group (A) RSWT and Group (B) phonophoresis demonstrated higher significant benefits, RSWT’s mechanical and regenerative effects made it more effective overall. Diclofenac phonophoresis remains a valuable option, particularly for patients with a primary goal of pain and inflammation management. |

| Summary, etc. | الخلفية: التهاب وتر عضلة فوق الشوكة المزمن يُعتبر من الأسباب الشائعة لألم الكتف. وعلى الرغم من شيوع هذه الحالة، فإن آلية حدوثها غير مفهومة بالكامل. الهدف: يهدف هذا البحث إلى دراسة ومقارنة تأثير العلاج بالموجات التصادمية الشعاعية مقابل تأثير انتقال العقاقيربالموجات فوق الصوتية باستخدام جل ديكلوفيناك الصوديوم في علاج التهاب وتر عضلة فوق الشوكة المزمن على سُمك وتر العضلة فوق الشوكة، شدة الألم في مفصل الكتف، مدى حركة مفصل الكتف، واستبيان وظيفة الطرف العلوي. التصميم: دراسة عشوائية محكمة. المواد والطرق: شارك في هذه الدراسة 69 مريضًا يعانون من التهاب وتر عضلة فوق الشوكة المزمن. تم تقسيم المشاركين إلى ثلاث مجموعات، تضم كل مجموعة 23 مريضً. المجموعة أ تلقت علاج الموجات التصادمية بقوة تردد الصدمة ۱۰ هرتز وكثافة طاقة 6۰۰۰ صدمة بين <br/>0.20، 0.35 ميلي جول/مم، لمدة 4 أسابيع جلسة واحدة في الأسبوع، بالإضافة إلى العلاج التقليدي. المجموعة ب تلقت علاج باستخدام جهاز الموجات فوق الصوتية + جل الكيتوبروفين في وضعية مستمرة، بتردد ۱ ميجاهرتز وقوة ٢ واط / سم، تم تطبيقه لمدة خمسة دقائق على كل منطقة، بالإضافة إلى العلاج التقليدي لمدة ٤ أسابيع، ثلاث جلسات في الأسبوع المجموعة ج هي مجموعة التي تلقت العلاج التقليدي فقط وهو عبارة عن (تمارين اطالة+ تمارين تقوية+ كمادات دافئة+التحفيز الكهربي عبر الجلد). القياسات: سمك وتر العضلة فوق الشوكة، شدة الألم في مفصل الكتف باستخدام مقياس البصري، وظيفة الطرف العلوي باستخدام استبيان داش ومدى حركة مفصل الكتف (الاختطاف، الثني، والدوران الخارجي) النتائج: أظهرت كل مجموعات المرضى تحسنا ملحوظا في جميع المتغيرات المقاسة. عند مقارنة المجموعتين، أظهرت مجموعة الموجات التصادمية تحسنا ملحوظاً خاصا في تقليل شدة الألم والمدى الحركي والقدرة الوظيفية للأطراف العلوية من مجموعة انتقال العقاقير بالموجات الصوتية. الخلاصة: قد اظهرت نتائج البحث وجود فارق ذو دلاله إحصائية في كل من المجموعات الثلاث بعد العلاج لكن العلاج بالموجات التصادمية الشعاعية كان له التأثير الافضل من العلاج بانتقال العقاقير بالموجات فوق الصوتية على التهاب وتر عضلة فوق الشوكة من حيث قلة سمك الوتر ,نقص المؤشر البصري لقياس الالم , نقص في نسبة استبيان وظائف الكتف و زيادة المدى الحركي للكتف في الثلاث حركات المذكورة اعلاه . |
