| Summary, etc. | Chronic kidney disease (CKD) is a worldwide health problem. Functional <br/>MRI techniques have been applied to assess early renal damage non-<br/>invasively and quantitatively. One of these techniques is diffusion-<br/>weighted magnetic resonance imaging which is sensitive for the detection <br/>of the combined effects of blood microcirculation and Brownian motion <br/>of water molecules, which is expressed as an apparent diffusion <br/>coefficient (ADC), within the tissues <br/>Aim of work: to analyze the role of diffusion weighted magnetic <br/>resonance imaging and apparent diffusion coefficient of the renal <br/>parenchyma in detection of different stages of chronic kidney disease and <br/>their relation to renal functions. <br/>Methods: A case control study was conducted on 80 patients: 40 with <br/>deranged renal function tests and other 40 healthy volunteers. DWI using <br/>a 1.5-Tesla MRI (at b-values of 0, 50 and 800s/mm2) was done. A <br/>circular region of interest of size 70-100mm3 was placed on the outer one <br/>third of the renal parenchyma at the upper, middle and lower renal poles <br/>of both kidneys, the mean ADC value of these 3 readings was then <br/>calculated for each kidney. Then the mean ADC for both kidneys of each <br/>patient was calculated. Finally, correlation was made with mean ADC <br/>value for the healthy participants (control group) and the mean ADC <br/>value for the patients with impaired renal functions (the cases group). <br/>Results: There was a significant reduction in ADC values in cases group <br/>when compared with control group. ADC can be used to discriminate <br/>between control group and Cases group at a cutoff level of > 1.8, with <br/>100% sensitivity, 85% specificity, 87% PPV and 100% NPV. ADC <br/>values progressively decreased with advancing CKD stages with a strong <br/>negative correlation was found between ADC values and serum <br/>creatinine and strong positive correlation with e-GFR. <br/>Conclusion: MRI ADC values can be used as non-invasive biomarkers <br/>for evaluating renal parenchymal dysfunction, with high sensitivity for <br/>discrimination of CKD stages. |

| Summary, etc. | يعتبر مرض الكلي المزمن مشكلة صحيه عالمية كبري. تم تطبيق تقنيات التصوير بالرنين المغناطيسي الوظيفية لتقييم الضرر الكلوي المبكر بشكل غير جراحي وكمي. إحدى هذه التقنيات هي التصوير بالرنين المغناطيسي بخاصية الانتشار، وهو حساس للكشف عن التأثيرات المشتركة لدوران الأوعية الدقيقة في الدم والحركة البراونية لجزيئات الماء، والتي يتم التعبير عنها بمعامل الانتشار الظاهري. داخل الأنسجة<br/>هدف العمل:<br/> تحليل دور التصوير بالرنين المغناطيسي الموزون للانتشار ومعامل الانتشار الظاهري لحمة الكلى في الكشف عن المراحل المختلفة لمرض الكلى المزمن وعلاقتها بوظائف الكلى<br/>أجريت دراسة على 80 مريضاً: 40 منهم يعانون من خلل في اختبارات وظائف الكلى و40 متطوعاً أصحاء. تم إجراء التصوير بالرنين المغناطيسي بقوة 1.5 تسلا بقيم تبلغ 0 و50 و800 ثانية/مم2. تم حساب متوسط قيمة معامل الانتشار الظاهري لكلتا الكليتين لكل مريض. وتم إجراء االمقارنة بين متوسط قيمة معامل الانتشار الظاهري للمشاركين الأصحاء مع المرضى الذين يعانون من اختلال وظائف الكلى.<br/>كان هناك انخفاض كبير في قيم معامل الانتشار الظاهري في المرضي الذين يعانون من اختلال وظائف الكلي بالمقارنة مع المشاركين الاصحاء. انخفضت قيم معامل الانتشار الظاهري تدريجياً مع تقدم مراحل مرض الكلى المزمن مع وجود علاقة سلبية قوية بين قيم معامل الانتشار الظاهري والكرياتينين في الدم وارتباط إيجابي قوي مع معدل الترشيح الكبيبي المقدر للكليز<br/>الاستنتاج:<br/> يمكن استخدام قيم ADC للتصوير بالرنين المغناطيسي كمؤشرات حيوية غير جراحية لتقييم الخلل الكلوي المتني، مع حساسية عالية للتمييز في المراحل المختلفة لمرض الكلى المزمن. |
