| Summary, etc. | Background :<br/>Redo bariatric surgery is an increasingly common procedure, primarily performed to address complications or suboptimal weight loss following initial bariatric operations. (195)<br/>There is an incidence of 20% of hiatus hernia to be an associated finding with bariatric surgeries. (196 , 197 )<br/>Accurate assessment of the gastric pouch size and the presence of hiatus hernia are critical for preoperative planning. <br/>Computed tomography (CT) volumetry has emerged as a potential solution to provide a more detailed assessment of the gastric pouch size and detect hiatus hernias with higher sensitivity. This will help in better preoperative assessment in redo bariatric surgeries<br/>Aim :<br/>Our aim was to detect the accuracy of CT in detection of hiatus hernia and measuring the gastric pouch volume in population undergoing re-do bariatric surgery.<br/>Methods <br/>we compared the findings of CT volumetry in patients undergoing redo bariatric surgery regarding gastric pouch and hiatus hernia with the intra operative findings .<br/>Results <br/>Our study demonstrated that CT accuracy in detection of gastric pouch volume and hiatus hernia is 96% .<br/>Conclusion <br/>Our study demonstrates that CT volumetry is a highly accurate and reliable method for assessing gastric pouch size and detecting hiatal hernia in patients undergoing redo bariatric surgery, |

| Summary, etc. | مازالت جراحات السمنه المفرطه فى نمو مستمر كطريقة علاج أولية للسمنة والأمراض المزمنة المرتبطة بالوزن, ومع استمرار زيادة عدد المرضى الذين خضعوا لعمليات السمنة الأولية تزداد أيضاً نسبة عمليات التصحيح.<br/>لقد كان الهدف من هذه الدراسة تحديد نسبة دقة الأشعة المقطعية ثلاثية الأبعاد قبل الجراحة فى الكشف عن فتق الحجاب الحاجز وحجم جيب المعدة فى المرضى الذين يخضعون لإعادة جراحة السمنة.<br/>الأسباب الرئيسية لمراجعة جراحة السمنة هى زيادة الوزن وارتجاع المرىء وإلتهابات المعدة وعدم تحسن الأمراض المزمنة المصاحبة للسمنة.<br/>تم إجراء دراستنا بعد فشل أنواع متعددة من جراحات السمنة الأولية ومنها تكميم المعده بالمنظار ورأب المعدة الرأسى أو تدبيس المعدة رأسياً ورباط المعدة القابل للتعديل بالمنظار.<br/>ولقد وصفت دراستنا أهمية استخدام الأشعة المقطعية ثلاثية الأبعاد قبل إجراء عمليات مراجعة فى المرضى التى فشلت لهم عمليات السمنة الأولية. وذلك لدقة الأشعة المقطعية ثلاثية الأبعاد فى الكشف عن فتق الحجاب الحاجز وحجم جيب المعدة فى المرضى الذين يخضعون لإعادة جراحة السمنة.<br/>حيث وجد إن مقارنة ما وجد فى أثناء إجراء العمليات المراجعة تتطابق بنسبة عالية مع نتائج الأشعة المقطعية. حيث وجد أن نسبة دقة الأشعة المقطعية ثلاثية الأبعاد فى تحديد حجم جيب المعدة كانت 96% أما بالنسبة للكشف عن فتق الحجاب الحاجز فكانت دقة الأشعة المقطعية ثلاثية الأبعاد 96%.<br/>هذه النتائج تؤكد أهميه إستخدام الأشعة المقطعية ثلاثية الأبعاد فى عمليات التصحيح حيث أنها تسمح للجراحين بالتخطيط الجيد لعمليات المراجعة. |
