| Summary, etc. | Aim: The aim of the study was to compare the effects of retraction and space closure using two different techniques on the amount of gingival display in the treatment of adult females with dentoalveolar protrusion and gummy smile. <br/>Methodology: Adult females patients above 19 years complaining of dento-alveolar protrusion and moderate gummy smile (3-7 mm) due to excessive vertical anterior dentoalveolar height which require upper premolar extraction and maximum anchorage were included in the study. The objectives of the study was to compare the effectiveness of using direct anchorage with infrazygomatic screw compared to indirect anchorage with interradicular screws and accentuated curve of spee archwires in improvement of gummy smile and protrusion and OHr-QoL using standardized smiling frames derived from videography, full face CBCT and OHIP-14 questionnaires. Both techniques were compared with regard to different horizontal and vertical smile variables, smiling and rest lip variables, amount of intrusion and root resorption on CBCT and OHrQoL pre-retraction, post-retraction and for retraction changes. <br/>Results: 18 adult females were included in the study and randomized equally to both intervention and comparator groups. A significant improvement of gingival display of upper anterior teeth in the intervention group (-2.1 mm to -3.34 mm) and comparator group (-1.69 mm to -2.97 mm) was achieved. A significant improvement in skeletal A-P relationship and soft tissue cephalometric variables were achieved in both groups, however the vertical relationship was only improved in the intervention group. A significant intrusion of anterior teeth in relation to the palatal plane in the intervention group (-1.93 mm to -2.96 mm) and comparator group (-1.85 mm to -2.38 mm) was achieved. As for root resorption, significant changes in root length in intervention (-2.38 mm to -3.35 mm) and comparator (-2.08 mm to 2.46 mm) groups were evident. <br/>Conclusions: Miniscrews have been shown to be effective in achieving significant retraction and intrusion of maxillary incisors, leading to marked improvements in smile and radiographic characteristics in adult females with maxillary dento-alveolar protrusion and gummy smile. However, treatment was associated with a significant root resorption in all anterior teeth. |

| Summary, etc. | الأهداف الرئيسية: هدفت هذه الدراسة إلى تقييم فعالية تقنيات مختلفة لاستخدام البراغي لتحريك الأسنان إلى الوراء وإغلاق الفجوات في علاج المرضى الذين تبرز أسنانهم وتظهر الكثير من اللثة عندما يبتسمون.<br/>الطرق: أجرينا دراسة مع نساء بالغات فوق سن 19 عامًا كانت تشعر بالقلق بسبب بروز الأسنان وظهور كمية معتدلة من <br/>اللثة عند الابتسام (٣-٧ ملم). كان هؤلاء المرضى بحاجة إلى خلع أسنان الضاحية العلوية وكانوا يحتاجون إلى ما يسمى "التثبيت القصوى". قمنا بمقارنة كيفية تحسين كل من التقنيتين في عرض اللثة وبروز الأسنان، بالإضافة إلى كيفية تأثيرها على جودة حياتهم المتعلقة بالصحة الفموية.<br/>ملخص النتائج: شملت الدراسة ١٨ امرأة بالغة، تم تقسيمهن بالتساوي بين طريقتي العلاج اللتين تم استخدامهما. أظهرت <br/>كل من المجموعتين تحسينات كبيرة في كمية اللثة المرئية عند الابتسام (عرض اللثة): من -١.٢ ملم إلى -٣٤.٣ ملم في مجموعة البرغي المباشر ومن -٧٩.١ ملم إلى -٩٧.٢ ملم في المجموعة الأخرى. أظهرت كل من المجموعتين أيضًا تحسينات في العلاقة بين عظام الفك والأنسجة الناعمة. شهدت كل من المجموعتين تحريك الأسنان الأمامية قليلاً نحو الأعلى نحو <br/>سقف الفم: من -٩٣.١ ملم إلى -٩٦.٢ ملم في مجموعة البرغي المباشر ومن -٨٥.١ ملم إلى -٣٨.٢ ملم في المجموعة الأخرى. ومع ذلك، عانت كل من المجموعتين أيضًا من بعض اختصار الجذور.<br/>الاستنتاجات الرئيسية: البراغي الصغيرة فعالة في تحريك الأسنان الأمامية العليا إلى الوراء وقليلاً نحو الأعلى في النساء اللاتي يعانين من بروز الأسنان وظهور الكثير من اللثة عند الابتسام. يؤدي هذا إلى تحسين كبير في ابتسامتهن وشكل وجوههن. ومع ذلك، قد يؤدي استخدام هذا العلاج إلى بعض اختصار الجذور لجميع الأسنان الأمامية. |
