| Summary, etc. | Background: Chronic Non-Specific Low Back Pain (CNSLBP) is one of the most common musculoskeletal disorders leading to postural disorders, and muscle problems. Purpose: This study aimed to investigate whether individuals with CNSLBP exhibit morphological changes in specific back muscles compared to healthy controls, and to determine the correlation between these changes and trunk extensor muscle strength. Design: Cross-sectional, correlation, and comparative analysis. Methods: Twenty-nine subjects with CNSLBP and 29 age matched healthy controls participated in this study. Their mean age was 32.31 ± 4.86 years. All subjects were assessed for function using Oswestry disability index (ODI), back muscle strength using a handheld dynamometer, and morphological changes of selected back muscles by an ultrasound imaging system. Results: Significant differences were observed between the CNSLBP and healthy groups in back muscle strength and several morphological parameters. Specifically, the CNSLBP group exhibited reduced back strength and alterations in the lumbar multifidus (LM), erector spinae (ES) and quadratus lumborum (QL) muscles, including decreased thickness, reduced Cross Sectional Area (CSA), and increased fatty infiltration. Back muscle strength showed positive moderate significant correlations with the LM thickness at both rest and contraction, with negative moderate correlation ES thickness asymmetry (at rest and during contraction), with negative moderate correlation the percentage change in right ES CSA, and with negative moderate correlation QL CSA asymmetry at rest, while correlations with other variables were non-significant. Conclusion: The observed group differences in muscle morphology and strength and the association of muscle morphology (LM, ES thickness, and QL CSA) with muscle strength underscore the potential contribution of these structural changes to the functional impairments seen in CNSLBP, highlighting the need for targeted rehabilitative interventions aimed at restoring muscle integrity and improving clinical outcomes |
