| Summary, etc. | BACKGROUND: Radial extracorporeal shockwave therapy (ESWT) <br/>proven by an animal study that it is effective in reducing post-<br/>laminectomy epidural fibrosis, might be similarly beneficial in managing <br/>the manifesting symptoms of post-laminectomy epidural fibrosis in <br/>humans. OBJECTIVE: This study aimed to investigate the effectiveness <br/>of radial ESWT on pain, lumbar range of motion (ROM), functional <br/>disability, F-wave minimal latency and F-chrondispersion in patients with <br/>post laminectomy epidural fibrosis. METHODS: Sixty patients of both <br/>genders with low back pain (LBP) and sciatica due to post laminectomy <br/>fibrosis aged from 30 to 50 years participated in this study. Patients were <br/>randomly divided into two equal groups (study and control groups). <br/>Patients in both groups received selected physical therapy program, while <br/>in the study group they received radial extracorporeal shock wave therapy <br/>(ESWT) in addition to selected physical therapy program. Measurement <br/>of pain intensity was assessed using visual analogue scale (VAS), pain <br/>pressure threshold and tolerance, lumbar ROM assessed by The BROM <br/>II, F-minimal Latency and chronodispersion for peroneal and tibial <br/>nerves, Oswestry disability index (ODI) were performed pre and post <br/>treatment. RESULTS: There were no statistically significant differences <br/>between both groups in pretreatment values of all measured variables. <br/>While there was a statistically significant decrease in the mean values of <br/>VAS score in both groups, ODI score in the study group only, F wave <br/>latency for both tibial and peroneal nerves in the study group only and F <br/>chondrodispersion for both peroneal and tibial nerves in the study group <br/>only and statistically significant increase in mean values of pain pressure <br/><br/><br/>III <br/><br/> <br/><br/>threshold in the study group only, pain tolerance in both groups, all <br/>lumbar ROM in the study group only post treatment. There were <br/>significant differences in all measured variables between both groups <br/>posttreatment favoring the study group. There was a significant positive <br/>correlation between VAS and F-chronodispersion scores for peroneal <br/>nerve and e significant negative correlation between side bending ROM <br/>(right &left) and F-minimal latency scores for peroneal nerve. <br/>CONCLUSION: It was concluded that radial ESWT could be a <br/>beneficial supplement to selected physical therapy program in reducing <br/>pain and functional disability and improving all lumbar ROM and nerve <br/>function in patients with post-laminectomy epidural fibrosis. So, it is <br/>highly recommended to be the main item in physical therapy program for <br/>those patients. |

| Summary, etc. | الخلفية: أثبتت الموجات التصادمية من خلال دراسة أجريت علي الحيوانات , انه فعال في تقليل التليف الناتج عن استئصال الصفيحة الفقرية. وقد يكون مفيداً بالمثل في تقليل الاعراض المصاحبة للتليف الناتج عن استئصال الصفيحه الفقرية في الانسان. الهدف: تهدف هذه الدراسة إلى دراسة تأثير الموجات التصادمية على الألم ، تحمل ضغط الألم ، تحمل الألم، المدي الحركي للفقرات القطنية، العجز الوظيفى ، زمن التأخر الأدنى ، فرق زمن الـتأخر الأدنى والأقصى في مرضي تليف ما بعد استئصال الصفيحة الفقرية. الطريقة: أجرى البحث علىستين مريضا من كلا الجنسين الذين تراوحت أعمارهم ما بين 30-50 عاما. حيث تم تقسيمهم عشوائيا إلى مجموعتين متساويتين (مجموعة الدراسة) التى تلقت الموجات التصادمية بالإضافة إلى برنامج العلاج الطبيعي االمختار، والمجموعة الضابطة التى تلقت فقط برنامج العلاج الطبيعى المختار. تم تقييم شدة الألم باستخدام مقياس التماثل البصري , قياس تحمل ضغط الألم , و قياس تحمل الألم بمقياس الضغط, . تم تقييم المدي الحركي للفقرات القطنية بواسطة جهاز قياس المدي الحركي للمنطقه القطنيه, و قياس العجز الوظيفى باستخدام استبيان أوسويسترى الخاص بالإعاقة و قياس زمن التاخر الأدني للعصب الظنبوبي و العصب الشظوي (F-wave) و فرق زمن التأخر الادني و الاقصي للعصب الظنبوبي و العصب الشظوي (F-chondrodispersion). وتم القياس لكلا المجموعتين قبل وبعد العلاج.<br/>النتائج: أوضحت المعالجة الإحصائية للنتائج عدم وجود فروق ذى دلالة إحصائية في متوسط قيم درجات جميع المتغيرات المقاسة بين المجموعتين قبل العلاج. بينما أسفرت النتائج عن وجود انخفاض ذو دلالة إحصائية في درجة قياس شده الألم في كلا المجموعتين ، درجة استبيان أوسويسترى الخاص بالإعاقة في مجموعة الدراسة فقط ، درجات زمن التأخر الأدنى و فرق زمن التأخر الادني و الاقصي للعصب الظنبوبي و العصب الشظوي في مجموعة الدراسة فقط وزيادات ذات دلالة إحصائية في متوسط قيم درجات تحمل ضغط الألم في مجموعة الدراسة فقط , تحمل الألم في المجموعتين ومدى حركة الفقرات القطنية في كل الاتجاهات في مجموعة الدراسة فقط بعد العلاج مع وجود فارق ذى دلالة إحصائية في جميع المتغيرات المقاسة بين المجموعتين بعد العلاج لصالح مجموعة الدراسة. أسفرت النتائج أيضا عن وجود علاقة طردية ذات دلالة إحصائية بين شدة الألم وفرق زمن التأخر الادني و الاقصي للعصب الشظوى وعلاقة وجود علاقة عكسية ذات دلالة إحصائية بين مدى الانحناء الجانبى للفقرات القطنية و زمن التأخر الأدنى للعصب الشظوى. <br/>الخلاصة: يعتبرالعلاج بالموجات التصادمية مكملا مفيدا لبرنامج العلاج الطبيعي المختار في تقليل الألم والعجز الوظيفي وتحسين المدي الحركي للفقرات القطنية ووظيفة العصب في مرضي تليف ما بعد استئصال الصفيحة الفقرية. ولذلك من الضروري أضافة الموجات التصادمية الي برنامج العلاج الطبيعي كوسيله أساسيه في علاج هؤلاء المرضي. |
