Summary, etc.	Background: Preterm prelabour rupture of membranes (PPROM) is the primary cause of <br/>preterm deliveries. It also increases the risk for infection, neonatal resuscitation, and <br/>respiratory distress syndrome (RDS). As well, PPROM leads to significant maternal <br/>complications as chorioamnionitis, puerperal infections, and operative delivery. Aim: this <br/>study aimed to examine the impact of applying Queensland Clinical Guidelines on pregnancy <br/>outcomes among women with PPROM. Design: A quasi-experimental design (Nonequivalent <br/>control group posttest- only design) was adopted. Setting: the study was conducted in the <br/>inpatient department of antenatal care at El-Galaa Teaching Hospital, Cairo. Sample: A total <br/>of 110 pregnant women having PPROM were recruited, a control group of 55 women were <br/>followed up first, and then 55 women of study group were enrolled to intervention. Tools: <br/>(A) Structured interview schedule; (B) Monitoring and follow up tool for PPROM; and (C) <br/>Evaluation of pregnancy outcomes sheet. Results: The rate of chorioamnionitis was lower in <br/>the study group (12.7%) versus the control group (34.5%), with a statistically significant <br/>difference (p = 0.007). Emergency cesarean section occurred in 43.6 % of the study group <br/>versus 63.6% of the control group (p = 0.004). Fetal complications were reported in 27.3% of <br/>the study group versus 63.6 % of the control group (p = 0.000). Neonatal sepsis occurred in <br/>1.8% of the study group versus 16.4% of the control group (p = 0.008); RDS in 21.8% versus <br/>52.7% (p = 0.001); NICU admission in 23.6% versus 50.9 % (p = 0.003); and neonatal death <br/>in 0.0% versus 10.9 % (p = 0.012), respectively, in the study group versus the control group. <br/>Premature birth occurred in 47.3 % of the study group versus 65.5% of the control group, <br/>showing a clinical difference but no statistical significance (p = 0.055). Conclusion: <br/>Application of queensland clinical guidelines for women with PPROM had a significant <br/>effect on reducing maternal, fetal, and neonatal complications. Recommendations: <br/>Integrating queensland clinical guidelines for PPROM as a main part of antenatal guidelines <br/>for care of women with PPROM.

Summary, etc.	الخلفية: يُعد الإنفجار المبكر لجيب المياه من الأسباب الرئيسية للولادة المبكرة، ويرتبط بزيادة مخاطر العدوى، وإنعاش حديثي الولادة، ومتلازمة الضائقة التنفسية. كما يؤدي الإنفجار المبكر لجيب المياه إلى مضاعفات كبيرة لدى الأم مثل التهاب المشيمة والسلى، والعدوى النفاسية، والولادة القيصريةالطارئة. الهدف: هدفت هذه الدراسة إلى تقييم تأثير تطبيق إرشادات كوينزلاند الاكلينيكية على نتائج الحمل لدى السيدات المصابات بالإنفجار المبكر لجيب المياه. التصميم: تم إعتماد تصميم شبه تجريبي (تصميم المجموعة الضابطة غير العشوائية بعد التطبيق). المكان: أُجريت الدراسة في قسم رعاية الحوامل الداخلي بمستشفى الجلاء التعليمي، القاهرة. العينة: شملت الدراسة 110 سيده حامل مصابة بالإنفجار المبكر لجيب المياه ، حيث تم أولاً متابعة مجموعة ضابطة مكونة من 55 سيدة، ثم تطبيق التدخل على مجموعة الدراسة المكونة من 55 سيده. الأدوات: (أ) استمارة جمع البيانات (ب) استمارة المتابعة (ج) استمارة تقييم نتائج الحمل. النتائج: انخفضت نسبة التهاب المشيمة والسلى في مجموعة الدراسة (12.7%) مقارنة بالمجموعة الضابطة (34.5%)، مع وجود فرق دال إحصائيًا (p = 0.007). كما حدثت الولادة القيصرية الطارئة في 43.6% من مجموعة الدراسة مقابل 63.6% من المجموعة الضابطة (p = 0.004). وسجلت المضاعفات الجنينية في 27.3% من مجموعة الدراسة مقابل 63.6% من المجموعة الضابطة (p = 0.000). كما ظهرت حالات التسمم بالدم الوليدي في 1.8% مقابل 16.4% (p = 0.008)، ومتلازمة الضائقة التنفسية في 21.8% مقابل 52.7% (p = 0.001)، والدخول إلى وحدة العناية المركزة لحديثي الولادة في 23.6% مقابل 50.9% (p = 0.003)، والوفاة الوليدية في 0.0% مقابل 10.9% (p = 0.012) على التوالي في مجموعة الدراسة مقارنة بالمجموعة الضابطة. أما الولادة المبكرة، فقد سُجلت في 47.3% من مجموعة الدراسة مقابل 65.5% من المجموعة الضابطة، مع وجود فرق سريري لكن دون دلالة إحصائية (p = 0.055). الاستنتاج: إن تطبيق إرشادات كوينزلاند الإكلينيكية لدى السيدات المصابات بالإنفجار المبكر لجيب المياه كان له تأثير واضح في تقليل المضاعفات لدى الأم والجنين وحديثي الولادة. التوصيات: يوصى بدمج إرشادات كوينزلاند الاكلينيكية ضمن الإرشادات الأساسية لرعاية الحمل للسيدات المصابات بالإنفجار المبكر لجيب المياه.
