520 #3 - SUMMARY, ETC.
Summary, etc. Background: Surgical repair in infancy is the main intervention for patients with congenital heart disease (CHD), who usually have associated malnutrition and growth retardation. Breast milk has a unique composition and good immune protection, which can effectively promote the growth and development of infants and accelerate the recovery of diseases.
Objective: To compare the effects of breast milk feeding and formula milk feeding on achieving appropriate growth velocity in infants who underwent cardiac surgery in the cardiac intensive care unit (ICU) and to compare the ICU hospital stay length and the feeding tolerance between the two groups
Methods: This study is a prospective observational study that has been carried out in the Cardiac intensive care unit in Cairo University Specialized Pediatric Hospital (CUSPH), during the period from January 2023 till October 2024. Infants diagnosed with congenital heart disease (CHD) underwent successful total repair of cardiac malformation by one stage operation in their first year of life then grouped into either breast milk feeding group (15 infant) or formula milk feeding group (15 infant) according to their original feeding method. The related clinical data and feeding effects were recorded and compared.
Results: Infants in the breast milk feeding group rather than formula milk group had favorable postoperative outcomes in the form of higher average daily weight (12.25 ± 2.75 versus 10.00 ± 2.28 gm/d; p value = 0.021), shorter duration of ICU stay (7.00 ± 2.04 d versus 8.80 ± 2.57 d; p value = 0.042) and shorter duration of mechanical ventilation (1.00 ± 0 versus 1.80 ± 0.92 d; p value = 0.022), also in breast milk group the infants started feeding at earlier time (22.53 ± 7.85 hours versus 34.73± 13.25 hours; p value = 0.011) and took shorter time to achieve full enteral intake (2.33 ± 0.62 days versus 3.87 ± 1.41 days; p value = 0.001). There is no statistically significant difference between the breast milk feeding group and the formula milk feeding group regarding preoperative and predischarge labs except for C-reactive protein (CRP) serum level which shows statistically significantly higher predischarge CRP in the formula feeding group (66.15 ± 38.11 versus 99.09 ± 43.63 mg/dl; p value = 0.036). In the breast milk feeding group, the predischarge albumin was statistically significantly higher than the postoperative albumin (3.83 ± 0.35 versus 4.09± 0.35 gm/dl; p value = 0.002), but this wasn’t the case within the formula milk feeding group. The incidence of complications such as feeding intolerance was lower in the breast milk feeding group than in the formula milk feeding group (6.7 % versus 46.7 %; p value = 0.035).
Conclusion: Breast milk feeding is effective with significant nutritional effect in infants with congenital heart disease after cardiac surgery. It is better than formula milk feeding, making breast feeding worthy for popularization and application in this group of infants
520 #3 - SUMMARY, ETC.
Summary, etc.
