| Summary, etc. | Aim of study: This study we sought to determine the relative effectiveness of <br/>the conventional TACE and Drug-eluting beads TACE (DEB-TACE) in the treatment of <br/>unresectable carcinoma of the liver. <br/>Patients and Methods: This prospective study included 75 patients with <br/>incurable hepatocellular carcinoma, split into two groups: 42 received cTACE <br/>treatment, and 33 received DEB-TACE treatment. This study intended to determine <br/>the relative effectiveness of the two techniques in the treatment of <br/>unresectable carcinoma of the liver. <br/>After 4-6 weeks post TACE the local tumor response to the treatment protocol <br/>was assessed on dynamic contrast study (triphasic CT or dynamic MRI) using the <br/>modified response evaluation criteria in solid tumors (mRECIST) based on the <br/>longest dimension of the viable tumor. <br/>Results: Considering the complication associated with the procedure (PES), <br/>there was only a statistically significant difference between the two groups according <br/>to nausea and abdominal pain (P-value=0.0126 and 0.0282 respectively). However, <br/>there was no difference between both groups regarding vomiting. <br/>According to mRECIST overall response after 1st session the results showed <br/>an objective response rate (ORR) of about 42.9 % vs. 42.4 % in the cTACE vs DEB-<br/>TACE arm, respectively. There was no statistically significant difference between <br/>the two groups according to local tumor response considering overall mRECIST (P <br/>value=0.998). <br/>Conclusions: <br/>Both procedures cTACE and DEB-TACE are effective in the management of <br/>intermediate stage HCC with no statistically significant superiority of DEB-TACE as well <br/>as fewer complications are noticeable with cTACE in comparison with DEB-TACE. |

| Summary, etc. | يعد سرطان الكبد (HCC) سادس أكثر أنواع السرطان انتشارًا على مستوى العالم ويحتل المرتبة الثالثة في الوفيات المرتبطة بالسرطان. بالنسبة للمرضى في المراحل المتوسطة أو المتقدمة، غالبًا ما تكون العلاجات المحلية والموضعية مثل حقن العلاج الكيماوي عبر الشرايين الكبدية (TACE) هي خيار العلاج الوحيد المتاح.<br/>التقنية الأكثر استخدامًا هي تقنية TACE التقليدية (cTACE)والتي يتم فيها حقن العلاج الكيماوي مباشرة بداخل شرايين الكبد، والتي تتبعها بعد ذلك اضافة جزيئات صغيرة مثل (إسفنجة الجيلاتين)والتي يتم حقنهابداخل الشرايين المغذيةللورم السرطاني ليتم حرمانه من الدم. ومن ثم تم تطوير حبات(DEBs) عن طريق تحميل جزيئات كحول البولي فينيل الدقيقة مع دوكسوروبيسين وكل هذه التقنيات للقضاء على السرطان بشكل موضعي ومنع تأثيرات هذه الادويةعلى بقية اجهزة الجسم.<br/>تشمل هذه الدراسة 75 مريضًا يعانون من سرطان الخلايا الكبدية المتقدم، مقسمين إلى مجموعتين: 42 تم علاجهم بـ cTACE، و33 تم علاجهم بـ DEB-TACE. يهدف هذا البحث إلى مقارنة فعالية كلا الطريقتين في علاج سرطان الكبد غير القابل للجراحة.<br/>بعد 4-6 أسابيع من TACE، تم تقييم فعالية العلاج موضعيا باستخدام دراسة التباين الديناميكي (التصوير المقطعي ثلاثي الأطوار أو التصوير بالرنين المغناطيسي الديناميكي) مع معايير mRECIST، مع التركيز على البعد الأكبر للورم الذي لايزال نطا.<br/>ونظرًا لتعقيدات الإجراء، أظهر الغثيان وآلام البطن فقط فروق ذات دلالة إحصائية بين المجموعتين (قيمة P = 0.0126 و0.0282، على التوالي). ومع ذلك، لم يكن هناك تمييز بين المجموعتين من حيث القيء.<br/>استنادًا إلى معايير mRECIST، أسفرت الجلسة الأولية عن معدل استجابة موضوعي (ORR) يبلغ حوالي 42.9% في ذراع cTACE و42.4% في ذراع DEB-TACE. لم يكن هناك تباين ملحوظ بين المجموعتين من حيث استجابة الورم الموضعية بناءً على mRECIST الشامل، بقيمة p تبلغ 0.998.<br/>ومن هذه الدراسة يمكن استنتاج ما يلي: <br/>كل من cTACE وDEB-TACE لهما نفس القدر من الفعالية في علاج المرحلة المتوسطة من سرطان الكبد، مع عدم وجود فرق كبير في التفوق بين الإجراءين. هناك مضاعفات أقل لوحظت مع cTACE مقارنة بـ DEB-TACE. |
