| Summary, etc. | Background: Internal rotation contracture (IRC) of the glenohumeral joint is the most common disabling sequela of obstetric brachial plexus palsy (OBPP), resulting from an imbalance between weak abductors and external rotators and relatively hyperactive adductors and internal rotators. Surgical intervention is often required to restore shoulder mobility and function. However, there is no consensus on the optimal surgical approach, timing, or specific indications.<br/>Methods: A systematic review and meta-analysis were conducted to evaluate the efficacy of soft-tissue reconstruction techniques for IRC in OBPP. A comprehensive search of PubMed, Cochrane Library, Scopus, and Web of Science identified studies published since 2000 that reported functional outcomes of shoulder surgery in BPBI patients. Extracted data included patient demographics, injury severity, previous surgeries, preoperative and postoperative range of motion (ROM) measurements, Mallet scale scores, glenohumeral remodeling parameters, and complication rates. Functional outcomes were assessed using aggregate Mallet scores, active and passive external rotation (ER) and abduction, as well as glenohumeral remodeling indices (glenoid retroversion and percent humeral head anterior alignment).<br/>Results: Both subscapularis sliding and arthroscopic release demonstrated efficacy in improving shoulder ROM and relieving contracture with low complication rates. Arthroscopic release provided superior ROM outcomes and a lower incidence of postoperative internal rotation weakness, whereas subscapularis sliding was more effective in promoting glenohumeral remodeling, particularly in cases of advanced dysplasia. The complication rate was lower for arthroscopic procedures compared to open techniques, though open procedures had a relatively low rate of avoidable complications.<br/>Conclusion: This study provides a comparative analysis of surgical interventions for IRC in OBPP, highlighting the advantages and limitations of each approach. Arthroscopic release is associated with superior functional outcomes and fewer complications, making it a preferable option for early-stage cases. Subscapularis sliding significantly contributes to glenohumeral remodeling and may be indicated for more advanced dysplasia. Future research should focus on refining surgical indications, standardizing outcome measures, and conducting prospective comparative studies to optimize treatment algorithms. Long-term follow-up is necessary to assess durability and refine evidence-based management strategies for IRC in BPBI patients. |

| Summary, etc. | التقلص الدوراني الداخلي لمفصل الكتف الحقاني العضدي هو أكثر المضاعفات المسببة للإعاقة شيوعًا لشلل الضفيرة العضدية الولادي، وينتج عن خلل في التوازن العضلي بين المبعدات والدوارات الخارجية الضعيفة من جهة، والمقربات والدوارات الداخلية مفرطة النشاط من جهة أخرى. ومن غير المرجح أن يتحسن التقلص أو وظيفة الكتف تلقائيًا دون تدخل جراحي، حيث تؤدي هذه الحالة عادةً إلى تقلصات عضلية ثابتة في العضلة تحت الكتف والعضلة الصدرية الكبرى، مصحوبةً بتيبس محفظي-أربطي محيطي يتطلب الإطالة أو التحرير الجراحي.<br/>حتى الآن، لا يوجد إجماع حول التقنية الجراحية المثلى التي تحقق أفضل النتائج من حيث تحسين حركة الكتف ووظيفته، كما أن المؤشرات الجراحية الواضحة لعلاج شلل الضفيرة العضدية الولادي لا تزال غير محددة بدقة. ويفضل التدخل الجراحي المبكر، لكن العمر الفاصل الأمثل لإجراء التدخل لم يُحدد بدقة في المراجع العلمية.<br/>وقد تم إجراء مراجعة منهجية وتحليل تلوي باستخدام المراجع العلمية المتاحة، حيث تم البحث في قواعد بيانات PubMed وCochrane Library وScopus وWeb of Science. تم تحديد جميع الدراسات المنشورة منذ عام ٢٠٠٠والتي تناولت نتائج علاج تقلص الكتف بإعادة البناء باستخدام الأنسجة الرخوة كعلاج لشلل الضفيرة العضدية الولادي، وتمت مراجعتها نصيًا بالكامل. وشملت الدراسة تحليل معايير الإدراج، البيانات الديموغرافية، الإجراءات الجراحية، النتائج الوظيفية، نوع الدراسة، ومستوى الدليل وفقًا لمركز أكسفورد للطب المبني على الأدلة.<br/>تضمنت عملية استخراج البيانات معلومات عن عمر المرضى، وشدة الإصابة، والتدخلات الجراحية السابقة، وقياسات مدى الحركة للكتف قبل الجراحة وبعدها، ودرجات مقياس Mallet قبل الجراحة وبعدها، بالإضافة إلى تشكل المفصل الحقاني العضدي وفترة المتابعة. ويتم تقييم النتائج الوظيفية لإعادة البناء بالأنسجة الرخوة في شلل الضفيرة العضدية الولادي باستخدام قياسات مدى الحركة، والأشعة، والمقاييس السريرية مثل مقياس Mallet. ولضمان تضمين أكبر عدد من الدراسات، تم اختيار المقياس الإجمالي لـ Mallet وجميع درجاته الفرعية، بالإضافة إلى قياسات الدوران الخارجي النشط والسلبي والمباعدة كمعايير للنتائج الوظيفية، بينما تم استخدام GRV وPHHA% كمؤشرات لإعادة تشكيل المفصل الحقاني العضدي. كما تم تحليل الدراسات لرصد أي مضاعفات بين المشاركين واستخراج هذه البيانات.<br/>أظهرت هذه المراجعة المنهجية والتحليل التلوي أن كلا التقنيتين الجراحيتين فعالتان في تحسين مدى الحركة وتخفيف التقلص، مع معدلات مضاعفات منخفضة. ومن المثير للاهتمام أن التقنية بالمنظار أثبتت فعالية أكبر في تحسين مدى الحركة، بينما كان الانزلاق تحت الكتف أكثر تأثيرًا في إعادة تشكيل المفصل الحقاني العضدي. كما تبين أن الطرق الجراحية بالمنظار تتميز بمعدل مضاعفات منخفض جدًا مقارنة بالتقنيات المفتوحة، التي قد تكون مصحوبة ببعض المضاعفات، لكنها تبقى عند معدل منخفض يمكن تجنبه. |
