| Summary, etc. | Background: Supraspinatus tears are a very common cause of pain, discomfort <br/>and dysfunction with a natural progressive course with aging. Many <br/>management modalities have been proposed to deal with supraspinatus tears <br/>ranging from conservative management to the more invasive approaches as <br/>arthroscopy and surgery. This study aims to evaluate the outcome of <br/>conservative management on patients with supraspinatus tears. <br/>Methods: A prospective case series was carried out on 20 patients with <br/>degenerative small to medium supraspinatus tears. Clinical outcomes were <br/>assessed upon presentation, at 3 months and 6 months using the Constant-<br/>Murley score (CMS) and the American Shoulder And Elbow Surgeons score <br/>(ASES). <br/>Results: The study population encompassed 14 females and 6 males with a <br/>mean age of 59.1 out of which 16 patients had a successful conservative <br/>management and 4 patients who chose operative management along the course <br/>of the study. Statistically significant improvement was observed in all outcome <br/>measures for the patients who followed the conservative management <br/>protocol. ASES score mean improved from 46.90 at presentation to 74.71 at 3 <br/>months and to 80.25 at 6 months and CMS improved from 50.70 at presentation <br/>to 73.29 at 3 months and to 78.56 at 6 months. <br/>Conclusion: The findings of this study suggest that conservative management of <br/>supraspinatus tears is a valuable option showing good clinical outcomes upon <br/>proper selection of patients particularly in patients who do not wish to undergo <br/>operative management, were unfit, with low demands or had minimal <br/>symptoms. |

| Summary, etc. | تعد تمزقات العضلة فوق الشوكة من الأسباب الشائعة جدًا لآلام الكتف وعدم الراحة والخلل الوظيفي، حيث تأخذ مسارًا تدريجيًا طبيعيًا مع التقدم في العمر. تم اقتراح العديد من الأساليب العلاجية للتعامل مع تمزقات العضلة فوق الشوكة، بدءًا من العلاج التحفظي وصولًا إلى التدخلات الأكثر توغلًا مثل التنظير الجراحي والجراحة. يهدف هذا البحث إلى تقييم نتائج العلاج التحفظي لدى المرضى الذين يعانون من تمزقات العضلة فوق الشوكة.<br/>المنهجية:<br/>تم إجراء دراسة مستقبلية لسلسلة حالات على 20 مريضًا يعانون من تمزقات تنكسية صغيرة إلى متوسطة في العضلة فوق الشوكة. تم تقييم النتائج السريرية عند التشخيص، وبعد 3 أشهر، وبعد 6 أشهر باستخدام مقياس كونستانت-مورلي (CMS) ومقياس جمعية جراحي الكتف والمرفق الأمريكية (ASES).<br/>النتائج:<br/>تكونت عينة الدراسة من 14 أنثى و6 ذكور، بمتوسط عمر 59.1 ± 4.08 سنة. من بين هؤلاء، نجح العلاج التحفظي في 16 مريضًا، بينما اختار 4 مرضى التوجه إلى العلاج الجراحي أثناء فترة الدراسة. لوحظ تحسن إحصائي ملحوظ في جميع مقاييس التقييم للمرضى الذين التزموا ببروتوكول العلاج التحفظي، حيث ارتفع متوسط ASES من 46.90 عند التشخيص إلى 74.71 بعد 3 أشهر وإلى 80.25 بعد 6 أشهر، في حين تحسن متوسط CMS من 50.70 عند التشخيص إلى 73.29 بعد 3 أشهر وإلى 78.56 بعد 6 أشهر.<br/>الاستنتاج:<br/>تشير نتائج هذه الدراسة إلى أن العلاج التحفظي لتمزقات العضلة فوق الشوكة يعد خيارًا فعالًا، حيث يحقق نتائج سريرية جيدة، لا سيما لدى المرضى غير الراغبين في الخضوع للجراحة أو غير المؤهلين لذلك. |
