Summary, etc. Background: Blount’s disease is a progressive disorder characterized <br/>by genu varum due to growth disturbances in the medial proximal tibial <br/>physis. While traditional methods such as osteotomy and guided growth <br/>with tension band plating (8-plate) have been widely used, recent evidence <br/>suggests that percutaneous epiphysiodesis using transphyseal screws <br/>(PETS) may offer superior outcomes with faster correction rates and fewer <br/>complications. <br/>Methods: This prospective interventional study evaluated the efficacy <br/>of PETS in treating juvenile and adolescent Blount’s disease. A total of 61 <br/>patients (80 knees) aged 4–12 years were treated at Abu Elreish Pediatric <br/>Hospital between January 2022 and January 2025. Radiological and clinical <br/>parameters, including mechanical axis deviation (MAD), correction rates, <br/>and functional outcomes (Böstman knee score), were assessed <br/>preoperatively, postoperatively, and at final follow-up. <br/>Results: The mean preoperative MAD improved significantly <br/>postoperatively, with a mean correction of 43.0° and a correction rate of <br/>3.66° per month (p < 0.001). Bilateral cases exhibited slightly higher <br/>correction values, but the differences were not statistically significant <br/>compared to unilateral cases. No significant differences were noted in <br/>outcomes based on gender or laterality. Complications were minimal, with <br/>a 9.8% reoperation rate and 3.3% rebound deformity, while no cases of <br/>overcorrection were observed. <br/>Conclusion: PETS is a promising alternative for hemiepiphysiodesis in <br/>juvenile and adolescent Blount’s disease, offering effective correction with <br/>a low complication rate. The technique demonstrated significant alignment <br/>improvements and functional recovery, making it a viable option for guided <br/>growth in pediatric patients with tibia vara.

Summary, etc. هدفت هذه الدراسة إلى تقييم فعالية تقنية المسمار عبر غضروف النمو في تصحيح تشوهات انحناء عظم الساق (مرض بلونت) لدى الأطفال والمراهقين. شملت الدراسة 61 مريضًا (80 ركبة) تتراوح أعمارهم بين 4–12 عامًا، خضعوا للجراحة في مستشفى أبو الريش للأطفال بين عامي 2022 و2025. اعتمدت الدراسة على قياسات انحراف المحور الميكانيكي (MAD) قبل وبعد الجراحة، بالإضافة إلى تقييم الوظيفة الحركية باستخدام مقياس بوستمان<br/>أظهرت النتائج تحسنًا كبيرًا في محاذاة الأطراف، حيث بلغ متوسط تصحيح الانحراف 43.0مم ، بمعدل تصحيح شهري ، دون فروق ذات دلالة إحصائية بين الحالات أحادية أو ثنائية الجانب، أو بين الجنسين. كانت المضاعفات محدودة (9.8% إعادة جراحة، 3.3% ارتداد تشوه)، مع عدم تسجيل أي حالات فرط تصحيح.<br/>مقارنةً بالتقنيات التقليدية (شريحة ال8 أو الدبابيس)، تفوقت تقنية المسمار عبر غضروف النمو في سرعة التصحيح وانخفاض المضاعفات، مما يجعلها خيارًا آمنًا وفعّالًا لعلاج تشوهات مرض بلونت، خاصةً لدى المرضى ذوي النمو المتبقي. تؤكد النتائج أهمية اعتماد هذه التقنية كخط أول في التصحيح الموجه للنمو، مع الحاجة لدراسات مستقبلية طويلة المدى لتأكيد استدامة النتائج.
