| Summary, etc. | Background: Fractures of the inferior pole of the patella are a unique type of patellar <br/>fracture which need surgical intervention if it is displaced or affect extensor <br/>mechanism of knee. Various techniques have been proposed for treating such <br/>fractures. Among these surgical techniques, tension band wiring and trans osseous <br/>reattachment may be the most commonly performed. <br/>Objectives: compare between the two modalities for fixation of inferior pole patella <br/>to reveal which of them has better clinical outcomes and less post-operative <br/>complication. <br/>Methods: Prospective randomized controlled clinical trial which include 60 patients <br/>with lower pole patellar fracture. 30 cases will be done with krackow sutures with <br/>high tensile strength sutures and 30 cases will be done with k-wires and tension band. <br/>The 2 groups underwent follow up through 6 up to 9 months period. <br/>Results: There was no significant difference between the 2 groups regarding age, <br/>gender, side and comminution, time to surgery, incidences of union time, nonunion, <br/>infection, ROM, visual Analouge Score, (VAS), bostman score and lyshlom score. <br/>While there were significant decreases of need to 2nd surgery in KT while less <br/>incidence of initial displacement in TBW technique. <br/>Conclusion: Both techniques are effective in treatment of lower pole patellar <br/>fracture. However, one technique is better in less initial displacement and the other <br/>don’t need to remove the implant |

| Summary, etc. | تمثل كسور الجزء السفلى لعظمة الرضفة من خمسة الى اثنان وعشرين فى المائة من كل كسور عظمة الرضفة والتى يمكن ان تعطل الالية المسؤولة عن انبساط عضلات الركبة والتى بدورها قد تؤدي الى اعاقة وظيفية واضحة .<br/>يوصى بالتدخل الجراحى فى حالات الكسور المتحركة للجزء السفلى من عظمة الرضفة لاستعادة الالية المسؤولة عن انبساط العضلات للركبة فى الطرف السفلى.<br/>تم اقتراح طرق عديدة لعلاج مثل هذه الكسور . من بين هذه الطرق الجراحية طريقة استخدام الاسلاك المعدنية والشدة السلكية و طريقة اعادة التثبيت بواسطه خيوط شديده الصلابة عبر انفاق فى العظام.<br/>ولذلك فان الهدف الرئيس من هذه الدراسة المستقبلية هي مقارنة بين الطريقتين لاكتشاف ايهما افضل من حيث النتائج العملية ونتائج الاشعات وايهما اقل مضاعفات بعد العملية. <br/>شملت هذة الرسالة ستون مريضا مقسمين الى مجموعتين. مجموعة كراكاو و مجموعة الاسلاك المعدنية والشدة السلكية . ثلاثون مريض لكل مجموعة. وكانت نتائج هذة الدراسة فى اخر فترة المتابعه والتى تتراوح ما بين ستة الى تسعة شهور كالاتى <br/>لا يوجد فرق مهم فى الدلائل الاحصائية بين المجموعتين فيما يخص :<br/>•السن والنوع والجانب وتفتت الكسر<br/>•الوقت اللازم لاداء الجراحة <br/>•نسبة انسال و سلفاتى في المتابعة الاخيرة<br/>•المدى الحركى للركبة وتقييم بوستمان وليشولوم ووتقييم الالم الوجهى <br/>يوجد فرق مهم فى الدلالات الحصائية بين المجموعتين كالاتى :<br/>•مدة العملية في مجموعة كاركاو اكبر من مجموعة الاسلاك المعدنية والشدة السلكية<br/>•فقدان الرد فى الكسر في مجموعة كاركاو اكبر من مجموعة الاسلاك المعدنية والشدة السلكية<br/>•الاحتياج الى رفع المعدة الصناعية في مجموعة الاسلاك والشدة لاسلكية اكبر من مجموعة كاركاو |
