| Summary, etc. | Aim: The aim of the clinical section of this study was to investigate the difference between single-piece and staged two-piece patient-specific titanium mandibular subperiosteal implants (SPIs) in terms of implant survival and soft tissue dehiscence. Concomitantly, a three-dimensional finite element analysis (3D FEA) study was conducted to explore the differences in biomechanical behavior of the aforementioned SPI designs. <br/>Materials and methods: The 3D FEA study involved two simulations: Simulation 1 represented oblique right-side loading representing the side that harbored the single-piece SPI framework, while simulation 2 represented oblique left-side loading representing the side that harbored the two-piece SPI framework. Peri-implant stresses, von Mises stress in frameworks, fixation screws, and overlying prosthesis as well as total deformation of fixation screws were assessed. The clinical study included 5 male patients with a severely atrophied mandible. They all received a unilateral single-piece SPI on one side, and a unilateral staged two-piece SPI on the contralateral side and then received a cement-retained prosthesis with a short cantilever on a total of 4 upstanding abutments. Implant survival and soft tissue dehiscence were the outcomes recorded after 3, 6 and 12 months. <br/>Results: 3D FEA results revealed that stress values at the peri-implant bone, on fixation screws as well as the total deformation of fixation screws were fairly comparable for both SPI designs. However, the stress values recorded for the framework in simulation 2 were considerably greater than those recorded for the framework in simulation 1. Clinical results revealed 100 % survival rate for the single-piece SPIs throughout the follow up period. For the staged two-piece SPIs, the survival rate was 80 % at the end of the 12-month follow-up period. For the single-piece SPIs, no dehiscence was observed at 3 and 6 months follow-up. However, 2 abutment posts showed dehiscence at the 12 month follow-up. For the staged two-piece SPIs, 2 abutment posts showed dehiscence with implant base exposure at the 6 month follow-up. However; no statistically significant differences were found between the two SPI designs regarding both implant survival and dehiscence among all follow-up periods. <br/>Conclusion: Within the limitations of the current study, it could be concluded that both SPI designs were comparable from a clinical perspective; however, the single-piece SPI design could be considered superior to the two piece SPI design from a biomechanical perspective. |

| Summary, etc. | تعد غرسات التايتانيوم المثبتة تحت السمحاق و التي تصمم خصيصا للمريض علاجا فعالا لمن يعانون من فقد كلي للأسنان مصاحب بضمور الفك السفلي. و قد ساهم التطور التكنولوجي في تسهيل عملية تصميم و تصنيع هذا النوع من الغرسات بشكل فعال و دقيق.<br/>تضمن الشق الأول من البحث اجراء تحليل العناصر المحدودة ثلاثي الأبعاد لمقارنة تصميمين مختلفين للغرسات المثبتة تحت السمحاق عن طريق تطبيق حمل مائل بزاوية 30 درجة على الجانب الأيمن لنموذج الفك السفلي و المثبت به غرسة تحت السمحاق مكونة من قطعة واحدة، ثم أعقبه تطبيق حمل مائل بزاوية 30 درجة على الجانب الأيسر لنفس النموذج و المثبت به غرسة تحت السمحاق مكونة من قطعتين. تم دراسة تأثير الأحمال على الغرسات و العظم المحيط بكل غرسة و البراغي المثبتة للغرسات، بالأضافة إلى تأثيرها على البِدْلَةٌ السِنِّيَّةٌ الثابتة.<br/>أظهرت نتائج تحليل العناصر المحدودة ثلاثي الأبعاد قيما متشابهة لكلا التصميمين من حيث أقصى جهد رئيسي عند العظم المحيط بهيكل الغرسة و جهد (von Mises) على براغي التثبيت، بالإضافة إلى قيم التشوه الشامل لبراغي التثبيت. و لكن، قيم جهد (von Mises) المسجلة لهيكل الغرسة في المحاكاة الثانية كانت ضعف القيم المسجلة لهيكل الغرسة في المحاكاة الأولى. <br/>في ظل محدوديات هذا البحث، يمكن استنتاج الآتي: أن التصميمين متشابهان من الناحية السريرية و لكن التصميم الاول (غرسة تحت السمحاق مكونة من قطعة واحدة) قد يفوق التصميم الثاني (غرسة تحت السمحاق مكونة من قطعتين) من الناحية الميكانيكية الحيوية.<br/>تضمن الشق الثاني من البحث تجربة سريرية عشوائية شارك فيها خمس مرضى (رجال)، حيث تم تثبيت غرسة تحت السمحاق مكونة من قطعة واحدة في جانب من الفك السفلي الضامر بالإضافة إلى غرسة تحت السمحاق مكونة من قطعتين في الجانب الآخر من الفك لكل مريض. لم تظهر النتائج فروق ذات دلائل إحصائية من حيث معدل بقاء الغرسات و كذلك الإنشقاق اللثوي. |
