| Summary, etc. | Bariatric surgery has proved to be an efficient intervention in the management <br/>of obesity. As a result, the number of post-bariatric surgery patients in the population <br/>is growing (King et al., 2016). <br/>Postoperative analgesia presents various challenges in vulnerable patient <br/>groups suffering from obesity. With the high potential risk of respiratory depression <br/>and postoperative pulmonary complications associated with opioid use, such as <br/>atelectasis and pneumonia, the availability of other pain management modalities is <br/>essential (Elgendy et al., 2020). <br/>Ultrasound-guided erector spinae plane block (ESPB) is a regional anesthesia <br/>technique in which a local anesthetic agent is injected deep to the erector spinae <br/>muscle into the fascial plane, allowing it to diffuse both cranially and caudally. <br/>This technique is intended to provide both visceral and somatic abdominal <br/>analgesia |

| Summary, etc. | مقدمة:<br/>تُعد السمنة وزيادة الوزن من المشكلات الصحية المتفاقمة عالميًا، وتمثل تحديًا كبيرًا للوقاية من الأمراض المزمنة. وقد أثبتت جراحات السمنة فعاليتها في معالجة هذه الحالة، مما أدى إلى تزايد عدد المرضى الذين خضعوا لهذه الإجراءات.<br/>يمثل تسكين الألم بعد جراحة السمنة تحديًا خاصًا، وخصوصًا لدى مرضى السمنة المفرطة، نظرًا لزيادة احتمال حدوث كبت تنفسي ومضاعفات رئوية عند استخدام الأفيونات، مثل الانخماص والالتهاب الرئوي. لذلك تزداد الحاجة إلى تقنيات بديلة للتسكين الآمن والفعال.<br/>الحصار العضلي الشوكي الموجه بالموجات فوق الصوتية (ESPB) هو تقنية تخدير إقليمي تعتمد على حقن المخدر الموضعي في المستوى اللفافي أسفل عضلة المنتصب الشوكي، بهدف تحقيق تسكين حشوي وجسدي للبطن. وتُعد هذه التقنية فعالة، وسهلة الأداء، وقصيرة المدة، وقد أثبتت نجاحها في العديد من الإجراءات الجراحية المختلفة.<br/>رغم ذلك، فإن تطبيق هذه التقنية في المرضى ذوي السمنة المفرطة يمثل تحديًا تقنيًا بسبب صعوبة تحديد العلامات التشريحية وعمق الأنسجة. كما أن جودة التصوير بالموجات فوق الصوتية قد تتأثر بزيادة سماكة الأنسجة، مما يضعف وضوح الصورة ويزيد من التشويش.<br/>وقد أظهرت بعض الدراسات فعالية هذه التقنية في تقليل الحاجة للأفيونات وتحسين السيطرة على الألم، إلا أن البيانات المتعلقة بتطبيقها على المرضى ذوي السمنة المفرطة جدًا لا تزال محدودة.<br/>الهدف من الدراسة:<br/>تهدف هذه الدراسة إلى مقارنة فعالية الحصار العضلي الشوكي ثنائي الجانبين الموجه بالموجات فوق الصوتية مع الترشيح الموضعي للبوبيفاكايين في مواقع أدوات الجراحة، من حيث استهلاك الأفيونات أثناء وبعد العملية والسيطرة على الألم خلال أول 24 ساعة بعد الجراحة لدى المرضى ذوي السمنة المفرطة جدًا. |
