| Summary, etc. | Eyes showing minimal PVR with break(s) located anteriorly can be managed successfully using pneumatic retinopexy, vitrectomy or scleral buckle, while eyes showing break(s) located posteriorly or significant PVR require vitrectomy with gas or oil tamponade (3). Specific gravities of almost all internal tamponading<br/>factors are usually less than the balanced saline. Hence, gas or oil bubble floats at highest position and presses the retina providing superior retinal tamponade. On the other hand, this is not that effective on the inferior retina in order to be pressed down enough to reach the pigment epithelium layer failing to be a tamponade |

| Summary, etc. | انفصال الشبكية الناجم عن التمزق يشمل سُمك التمزق الشبكي بالكامل مع تراكم الزجاجي السائل تحت الشبكية، مما يؤدي إلى فصلها. التليف الزجاجي الشبكي هو نوع من الاستجابة الشفائية لحدوث تمزق في الشبكية. تبدأ سلسلة التهابية داخل العين نتيجة لانهيار الحاجز الدموي الشبكي، مما يؤدي إلى تكاثر الخلايا وإعادة تشكيل المادة خارج الخلوية. العوامل الالتهابية والتليّفية (مثل عوامل النمو، السيتوكينات والخلايا) تتسرب إلى كل من السائل الشبكي وتجويف الجسم الزجاجي، مسببة تليفًا فوق الشبكية وداخلها وتحته، مما يؤدي إلى اعتلال الجسم الزجاجي الشبكي التكاثري وانفصال شبكي متكرر.<br/>هدف الدراسة:<br/>قارنت الدراسة الحالية بين ازالة الجسم الزجاجي مع حزام الصلبة مقابل ازالة الجسم الزجاجي مع زيت السيليكون الثقيل في علاج انفصال الشبكية الاولي المصحوب باعتلال الجسم الزجاجي الشبكي التكاثري السفلي من حيث النجاح الاولى بعودة الشبكية الي مكانها والنجاح الثانوي بتحسن حدة الابصار.<br/>المرضى والوسائل:<br/>أُجريت دراسة مقارنة عشوائية على ثلاث وثلاثين عينًا لثلاثة وثلاثين مريضًا يعانون من باعتلال الجسم الزجاجي الشبكي التكاثري من الدرجة الثالثة في الجزء السفلي، المصاحب لانفصال الشبكية الأولي. تم علاج ست عشرة عينًا باستئصال الجسم الزجاجي مع زيت السيليكون الثقيل، وتم تصنيفهم كمجموعة الزيت الثقيل؛ وسبع عشرة عينًا خضعت للجراحة المشتركة بين استئصال الجسم الزجاجي مع حزام الصلبة ، وتم تصنيفهم كمجموعة تثبيت الصلبة مع استئصال الزجاجية.<br/>مقومات الدراسه تشمل:<br/>هدفنا كان تحقيق إعادة الالتصاق التشريحي للشبكية في غياب الحشو داخل العين. تم إزالة زيت السيليكون بعد 8 أسابيع في كلا المجموعتين. تم تقييم جميع المرضى من حيث أفضل حدة بصر مصحح وضغط العين في نهاية الأسبوع الأول والشهر الأول والثاني في العيون المملوءة بالزيت؛ وكذلك في نهاية الشهر الأول والثالث بعد إزالة زيت السيلسكون. |
