| Summary, etc. | Background: <br/>Shoulder pain is one of the most common musculoskeletal conditions <br/>encountered in outpatient clinics. It affects daily activities and may lead to <br/>movement restrictions due to various underlying conditions, such as <br/>adhesive capsulitis (AC), rotator cuff (RC) tears, tendinopathy, and gleno-<br/>humeral osteoarthritis. AC presentation which is far from specific and <br/>mimics several other painful shoulder disorders. <br/>Objectives and aim of the work: The aim of this work is to evaluate the <br/>role of ultrasonography in the diagnosis of adhesive capsulitis in painful <br/>shoulder compared to MRI in order to improve the AC’s diagnostic <br/>approach and the overall diagnostic performance of ultrasound in different <br/>shoulder pathologies. <br/>Methods: <br/>The study included 25 adult patients, with a mean age of 46.4 ± 10.7. Cases <br/>included 8 males (32%) and 17 females (68%). Clinical evaluation and <br/>assessments of the shoulder joint were done, as well as ultrasound and MRI. <br/>Clinical positive findings and radiological signs and parameters were <br/>recorded. The US diagnosis was correlated and compared with clinical and <br/>MRI diagnoses. <br/>Results: <br/>The correlation between clinical, ultrasound, and MRI evaluations <br/>demonstrated varying sensitivities, specificities, and diagnostic accuracies in <br/>diagnosing different causes of shoulder. US showed a reasonable diagnostic <br/>accuracy comparing to MRI by combination of static parameters including <br/>(CHL thickness, ICR thickness... etc.) and dynamic parameters as limited <br/>abduction and external rotation. US can also exclude other causes of painful <br/>stiff shoulder which mimics AC such as bursitis, RCT disease, biceps <br/>tendinopathy and AC joint disease. <br/>Conclusion: US role in diagnosis of AC is promising but lack of consensus <br/>on the used measurements and techniques for evaluation of AC is still a <br/>main limitation. US can be used as initial diagnostic tool as it is an available, <br/>less expensive and non-invasive tool. It also has the added benefit of <br/>dynamic imaging, hands-on contact with the patient and the lack of ionizing <br/>radiation. |

| Summary, etc. | التهاب المحفظة اللاصقه و حالة مُنهكة تتميز بألم مُتزايد وتقييد في نطاق حركة مفصل الكتف. يُصيب هذا المرض بشكل رئيسي النساء في منتصف العمر، لا يزال سبب المرض غير مؤكد ويُشير بعض الدلائل إلى ارتباطه ببعض الأمراض الجهازية، بما في ذلك داء السكري. يُمكن أن يُؤدي التشخيص والعلاج المُبكر لالتهاب المحفظة اللاصق إلى تحسن ملحوظ في الحالة المرضية.<br/>يهدف هذا العمل إلى تقييم دور الموجات فوق الصوتية في تشخيص التهاب المحفظة اللاصق في الكتف المؤلم مقارنة بالرنين المغناطيسي من أجل تحسين البرنامج التشخيصي لهذا المرض و أيضا الأداء التشخيصي للموجات فوق الصوتية في أمراض الكتف المختلفة.<br/>تضمن هذا البحث خمسة وعشرين مريضًا (ذكورًا وإناثًا) ممن يُشتبه بإصابتهم بالتهاب المحفظة اللاصق بعد الفحص الطبي. خضع كل مريض لفحص بالموجات فوق الصوتية علي مفصل الكتف المؤلم، و تمت المقارنة بالرنين المغناطيسي كونه وسيلة نشخيصية أساسية في أمراض الكتف المختلفة.<br/>لقد استنتجنا أنه يمكن استخدام الموجات فوق الصوتية كأداة تشخيصية أولية لأنها تعتبر وسيلة سهلة، أقل تكلفة، و غير تداخلية، من مميزاتها القدرة علي الفحص المتحرك بالإضافة الي التواصل مع المريض أثناء الفحص و عدم التعرض للإشعاع الضار.<br/>تتمتع الموجات فوق الصوتية بدقة تشخيصية مقبولة مقارنةً بالتصوير بالرنين المغناطيسي، وذلك من خلال الجمع بين عدد من المعايير الثابتة، بما في ذلك (سمك الرباط الغرابي العضدي، وسمك تجويف المحفظة السفلي ، إلخ) والمعايير الديناميكية ( مثل القصور في الاختطاف والدوران الخارجي) . إن دور الموجات فوق الصوتية في تشخيص واعد ولكن عدم وجود توافق في الآراء بشأن القياسات والتقنيات المستخدمة لتقييم التهاب المحفظة اللاصق لا يزال يشكل قيدًا رئيسيًا.<br/>تستطيع الموجات فوق الصوتية أيضا استبعاد الأسباب المسببة لآلام و تيبس الكتف التي تُحاكي التهاب المحفظة اللاصق ومنها التهاب الجراب، وأمراض وتر الكفة المدورة، واعتلال أوتار العضلة ذات الرأسين، ومرض المفصل الأخرمي الترقوي |
