| Summary, etc. | Objective: AFRS is non-invasive fungal sinusitis recognized in 1976, <br/>which is a subset of polypoid CRS that is characterized by the presence of <br/>eosinophilic mucin with non-invasive fungal hyphae within the sinuses and a <br/>type I hypersensitivity to fungi. The aim of this study is to evaluate serum <br/>IgE, eosinophils, basophils, Lund-Mackay score and Kupferbergs staging <br/>system as the predicting factors of early recurrence of AFS. <br/>Methods: This prospective study includes 84 patients with AFS presented <br/>to the Otolaryngology outpatient clinic between May 2023 and february <br/>2025, treated at department of Otorhinolaryngology and Head and neck <br/>surgery, Cairo University. Analysis was done for demographics, clinical <br/>presentation, co-morbidities, previous endoscopic sinus surgery, CT Lund-<br/>Mackay score pre-operative and follow up laboratorial tests (total serum <br/>IgE, serum eosinophil and serum basophil). Also Kupferburg endoscopic <br/>follow up score and pattern of sinus recurrence. <br/>Result: In present study, AFRS was more commonly seen in females, 51 <br/>participants (60.7%) and mean age was 26.3. Clinical manifestations <br/>distribution among participants were; discharge (100%), headache (73.8%), <br/>nasal obstruction (72.6%), hyposmia (29.8%), anosmia (53.6%) and eye <br/>symptoms (4.8%). Twenty two cases(26%) have done FESS before, (10.7%) <br/>have asthma and 52.3% have unilateral disease. Lund Mackay score average <br/>was 13.67. Serum IgE is elevated in most cases with significant drop in the <br/>first follow up same as eosinophils and basophils. Endoscopic follow up <br/>using kubferberg score is very important. 35.7% had recurrence, the most <br/>common recurrent sinuses were frontal (31.0%). Statistical significance was <br/>found regarding pre-operative Lund-Mackay score, kubferberg score and eye <br/>symptoms in relapsing cases presence of Asthma and time interval <br/>preoperative. IgE, eosinophil and basophils were high in relapsing group <br/>without clinical significance. <br/>Conclusion: AFRS is difficult to control with high rate of recurrence, <br/>endoscopic follow up is very important. Asthma is risk factor for AFRS, pre-<br/>operative Lund-Mackay score is indicator of recurrence. Although seum IgE, <br/>eosinophil and basophils are elevated in relapsing group, it is not statistically <br/>significant. |

| Summary, etc. | شملت هذه الدراسة 84 مريضا شملت هذه الرسالة 84 مريضا. أظهرت هذه الدراسة معدل تكرار مرتفع لفطريات الانف التحسسية ، بنسبة 35.7%. لا توجد حاليًا طريقة معتمدة للتنبؤ بتكرار الإصابة المرض. يُعد التنبؤ بالتكرار أمرًا بالغ الأهمية لتحقيق أقصى قدر من نجاح العلاج وتقليل خطر الانتكاس. نهدف في دراستنا إلى التنبؤ بالتكرار المبكر لفطريات الانف التحسسيه باستخدام البيانات المتاحة من المظاهر السريرية، والفحص بالمنظار، والتصوير، والفحوصات المعملية.<br/>يجب أن يصاحب العلاج متابعة جيدة للكشف عن المرضى الأكثر عرضة للانتكاس. عادةً ما يكون فطريات الانف التحسسية مرضًا أحادي الجانب، ويرتبط ارتباطًا وثيقًا بالربو الصدري. يُعد ارتفاع مستوى الاجسام المضادة IgE في الدم وزيادة كرات الدم البيضاء الحامضيه من السمات المميزة لـفطريات الانف التحسسية، مع انخفاض ملحوظ خلال فترة قصيرة بعد الجراحة.<br/>يُعدّ ارتفاع تقييم الاشعه المقطعيه (Lund-Mackay score) قبل الجراحة عاملًا مُنبئًا بتكرار المرض. يُفترض أن يُثير ارتفاع مستوى IgE في المصل بعد الجراحة شكوك الانتكاس.<br/>هناك أهمية كبيرة للأعراض التي تظهر بعد العملية الجراحية، مثل تغيرات حاسة الشم، والانسداد، وأعراض العين، والتي تعد عوامل تنبؤية لتكرار حدوث المرض.<br/>يُعدّ الجيب الجبهي والغربالي أكثر المناطق شيوعًا لتكرار الإصابة، يليهما الجيب الوتدي. تُعد المتابعة التنظيرية المنتظمة في العيادات الخارجية بالغة الأهمية لأنها تسبق ظهور الأعراض، ويمكن لتعديل العلاج أن يجنّب تكرار الإصابة وجراحة المراجعة.<br/>في دراستنا، لم نتمكن من إثبات أهمية IgE والخلايا الحمضية والقاعدية كعوامل تنبؤ بتكرار الإصابة، لكننا أثبتنا أهمية تقييم الاشعه لوند-ماكاي، والفحص الأنفي المنتظم بالمنظار، والأعراض الظاهرة بعد الجراحة كعوامل تنبؤ. كما أن الربو وطول الفترة الزمنية بين الأعراض والجراحة يزيدان من خطر الانتكاس. |
