Summary, etc. Background: <br/>Hyperbilirubinemia occurs commonly in neonates and is usually mild and <br/>transient, with no long-lasting sequelae. Transient Evoked Otoacoustic Emission <br/>(TEOAE) is served as the initial screening in the first day of life. In newborns that <br/>did not meet pass criteria TEOAE was repeated in 10-day period. If the second <br/>test did not meet pass criteria again, the screening was completed with auditory <br/>brainstem response (ABR). <br/>Aim and objectives: <br/>The main aim of this study was to determine the incidence of hearing <br/>impairment with hyperbilirubinemia and to identify low risk and high-risk groups <br/>and to identify other associated risk factors. <br/>Subjects and methods: <br/>This case-control study was conducted on 35 neonates with <br/>hyperbilirubinemia admitted to Neonatal Intensive Care Unit, Abo Elreish <br/>hospital, Cairo University in addition to 35 matched healthy neonates served as <br/>control group. <br/>Result: <br/>there was statistically significant relation between failed group and pass <br/>group regarding mode of delivery. there was statistically significant relation <br/>between failed group and pass group regarding mode of delivery, Platelet count <br/>and BIND Score. there was statistically significant relation between failed group <br/>and pass group regarding, BIND Score and TSB on admission. there was <br/>statistically significant negative correlation between AABR and Mode of delivery <br/>and OAE. <br/><br/><br/>there was statistically significant negative correlation <br/>between OAE and AABR, while there was statistically <br/>significant positive correlation between OAE and Mode of <br/>delivery and Platelet count. <br/>Conclusion: <br/>The mode of delivery and platelet counts, along with the <br/>severity of bilirubin- induced neurologic dysfunction, could be <br/>important considerations when evaluating hearing in infants <br/>with hyperbilirubinemia. The results of this study underline <br/>the importance of the Automated Auditory Brainstem <br/>Response and Otoacoustic Emissions Tests as efficient tools <br/>for monitoring the auditory brainstem pathway in neonates <br/>who are at risk of neurotoxicity and for diagnosing the earliest <br/>stages of auditory damage caused by high levels of bilirubin.

Summary, etc. فرط بيليروبين هو حالة شائعة عند الاطفال حديثي الولادة، وتتميز بتغير لون الصلبة و الجلد والاغشية المخاطة الي اللون الاصفر. وبما ان البيليوبين قابل للزوبان في الدهون فانة يمكن ان يمر عبر حاجز الدم في الدماغ وبيتوزع في الدماغ.<br/>يتغير البيلروبين غير المباشر وهو سبب الرئيسي لليرقان الوليدي شديد السمنة العصبية لجهاز العصبي المتخلف، خاصة عندما يتجاوز تركيز البيليروبين غير المباشر قدرة ربط الالبومين. ان ظاهرة البيليروبين المترسب الغير المباشر في العقد القاعدية وكزلك في نواة القوقعة الدهليزية تسبب متلازمة عصبية تسمي البرقان النووي بالاضافة الي فقدان السمع الحسي العصبي.<br/>ما يصل الي 40 % من الاطفال حديثي الولادة المصابين باليرقان معرضون لخطر فقدان السمع الاسباب اكثر شيوعا لفرط بيلبروبين الدم المرضي غير المباشر في زيادة انتاج البيليروبين بسبب الامراض الائحلالية التي تشمل انحلال الدم المناعي عدم توافق غشاء الخلية الحمراء الموروثة.<br/>(كثر الكربات الحمر)، ونقص انزيمات كرات الدم الاحمراء والاثنان كما يمكن ان يحدث فرط بيلبروبين<br/>الدم بسبب انخفاض التصفية( متلازمات كربجلر نجار وجيلبرت) وزيادة الدورة الدموة المعدية الكبدية مثل برقان الرضاعة الطبيعية.<br/>تعد استجابة جذع الدماغ السمعي المقياس الاكبر حساسية لتقييم القوقعة الصناعية الرجعية فضلا عن سلامة المسار السمعي ومع ذلك ، فان استخدام هذا القياس في حالات فرط بيليروبين الدم عند الاطفال حديثي الولادة امر مثي للجدل الي حد كبير لان العلاقة بين مستويات البيليروبين في الدم و التغيرات السمعية لاتزال مثير للجدل.
