Summary, etc.	Background: Axial bone sarcoma mainly Osteosarcoma (OS) and Ewing <br/>sarcoma (ES) are rare and aggressive tumors with a worse prognosis than <br/>tumors of extremities, this is due to different factors: the unique anatomy <br/>that render the ability of an adequate surgical excision and the poor <br/>tolerability of the spine and pelvic structures to high doses of <br/>radiotherapy. The data in literature on pediatric axial bone sarcoma is <br/>limited. <br/>Purpose: To assess the outcome of patients with axial OS and ES and to <br/>determine the different prognostic factors and its association with overall <br/>survival (OS) and event free survival (EFS). <br/>Methods: This is a retrospective cohort study including patients with <br/>axial ES and OS presented to and treated at Pediatric Oncology <br/>Department, National Cancer Institute, Cairo University from January <br/>2008 to December 2020. <br/>Results: Over the period of the study, a total of 40 patients were <br/>identified, 34 patients had ES and 6 patients had OS. For cases of ES, the <br/>median follow-up period was 37 months (ranged 0.4-164), the median <br/>OS was not reached. The cumulative overall survival at 1 year was 85.3 <br/>%, at 3 years was 54.7%, and at the end of the study was 54.7 %. The <br/>only significant clinical variable affecting outcome was the extent of <br/>disease at presentation. Patients with metastatic disease had a worse 3-<br/>year EFS (22.2%) compared to those with localized disease (67.4%), with <br/>a p value of 0.004, and a worse OS of 31.3 % compared to 71.8% for <br/>localized disease, with a p value of 0.003. For cases of OS, the median <br/>follow-up period was 12.9 months (ranged 3.2-44), the cumulative OS <br/>was 66.7 % at 1 year, and 16.7% at 3 years and at the end of the study. <br/>EFS at 1 year was 16.7%. <br/>Conclusion: Patients with axial OS and ES showed very poor survival <br/>rates with the presence of metastatic disease at presentation being the <br/>most significant prognostic factor in patients with Ewing Sarcoma.

Summary, etc.	هذه دراسه شملت مرضى الاورام العظميه المحوريه فى الأطفال بالمعهد القومى للأورام بجامعه القاهره فى الفتره ما بين يناير 2008 الى ديسمبر 2020 وتهدف الى تقييم فرص البقاء كما تهدف الى تحديد عوامل التنبؤ فى مرضى اورام العظام المحوريه من نوعى ساركوما العظام و الساركوما من نوع ايونج .<br/>شملت الدراسه المرضى تحت عمر 18 عاما الذين تم تشخيصهم و علاجهم حديثا بساركوما عظميه محوريه عاليه الدرجه, وقد تم استبعاد المرضى الذين قد تلقوا اى علاجات سابقه للاورام او كانت سجلاتهم المرضيه غير كامله وقد قمنا بتجميع البيانات من السجلات المرضيه وتشمل المعلومات الديموغرافيه والعرض السريرى ونتائج الاشعات ومدى الاستجابه للعلاج.<br/>لقد قمنا بتعريف مؤشرات البقاء كالتالى:<br/>البقاء الخالى من الاحداث : من التشخيص حتى حدوث زياده او ارتداد او وفاه من اى سبب.<br/>البقاء الكلى : من التشخيص حتى الوفاه من اى سبب او اخر متابعه.<br/>تلقى مرضى ساركوما ايونج علاج كيماوى تبعا لبرتوكولات مختلفه قبل وبعد 2016 يشمل VDC/IE اما بالنسبه لمرضى ساركوما العظام فقد تلقوا علاجا طبقا لبروتوكول EURAMOS<br/>شملت المجموعه 40 مريضا ؛ 34 منهم مصابون بساركوما ايونج و6 بساركوما العظام . بالنسبه للمرضى المصابون بساركوما ايونج فقد لوحظ زياده فى عدد الذكور على الاناث (56% : 44%) بمتوسط عمر بلغ 12.5 عام للجميع وقد لوحظ وجود نسبه عاليه من الثانويات المرضيه اغلبها الى الرئه <br/>وقد تلقى جميع المرضى العلاج الكيماوى وحقق 44 % منهم استجابه كامله وبلغت نتائج البقاء الكلى 85.3% عند السنه الاولى و 54.7% بعد ثلاث سنوات , مع وجود عامل تنبؤ رئيسى الا وهو وجود الثانويات كعامل عكسى لنسب البقاء.<br/>اما بالنسبه لساركوما العظام فقد كان لدينا 6 مرضى جميعهم مصابون باورام بالحوض وكانت النتائج متدنيه للغايه حيث توفى 83 % من المرضى بنهايه الدراسه وهذه النتائج تسلط الضوء على الحاجه الى استراتيجيات جديده للعلاج والمتابعه كما تنصح بضروره التركيز على الأكتشاف المبكر للمرضى وتحويلهم العاجل الى مراكز متخصصه لعلاج الاورام بما يضمن تحسين فرص البقاء لهؤلاء المرضى.
